LPN - Rensselaer/Albany Counties full time WEEKDAYS 8am to 4pm (NO mandatory overtime!)
The Community Hospice has a wonderful opportunity for a full-time LPN to work as a member of our Hospice Home Care Team caring for patients and families residing in either parts of Rensselaer county and east Albany county.

What you will do:
Coordinate closely with our RN Supervisor and RNs to provide nursing and personal care services as a part of a comprehensive plan of care to provide added support to patients and families
This is an ideal opportunity for someone who enjoys working closely with a team (our teams include RNs, Aides, medical directors with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers).
Training provided in end of life care.
What you will need:
Valid NYS LPN License
Minimum 1 year LPN experience required
Prior hospice/ home care experience preferred, but not required
Strong computer skills
Organization and communication skills and ability to work as part of a team

WORK SCHEDULE IS MONDAY TO FRIDAY 8:00am to 4:00pm. No On-Call, No Weekends, No Mandated Overtime
Pay Range: $24.00-$30.10 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
