Position: Foster Care Case Manager
Position Summary: Provides individual and family case management services to therapeutic cases as well as services to youth individually for the purposes of skill development around independent living, coping, and socialization. Works in partnership with the clinician for “special” or “exceptional” cases or manages case dynamics for “regular” rate cases. Case Manager meets regularly with clinicians and clinical director for case oversight. Provides documentation included but not limited to FASP’s, PHR, court reports, correspondence, and progress notes. The Case Manager is responsible for maintaining all activities in the case in the absence of the Clinical Worker under the close supervision of the Clinical Director.

Minimum Education and/or Professional Qualifications/Skills: One required
Masters Degree in Human Services related field, no experience.
Bachelor’s Degree in Human Services related field, with two years experience
Associates Degree with three to four years experience.

Job Title: Case Manager Department: Foster Care/SILP Title of Immediate Supervisor: Clinical Director POSITION SUMMARY: Provides individual and family case management services to therapeutic cases as well as services to youth individually for the purposes of skill development around independent living, coping, and socialization. Works in partnership with the clinician for “special” or “exceptional” cases or manages case dynamics for “regular” rate cases. Case Manager meets regularly with clinicians and clinical director for case oversight. Provides documentation included but not limited to FASP’s, PHR, court reports, correspondence, and progress notes. The Case Manager is responsible for maintaining all activities in the case in the absence of the Clinical Worker under the close supervision of the Clinical Director. TYPICAL RESPONSIBILITIES/DUTIES: ▪ To serve as a member of the treatment team with the clinical worker with responsibility to facilitate the child’s emotional care and monitor clothing needs and to provide social therapeutic activities and development through individual and group experiences. ▪ To provide experiences of socialization and connectedness to children in care such as facilitating life skill experiences which lead to the mastery of independence. ▪ To foster visitation and reunification of child and natural and/or fictive family and/or SILP youth. ▪ To be on call in case an emergency develops as per the crisis matrix. ▪ To provide services in the absence of the co-workers due to vacations, etc. ▪ Will take a learning stance relative to professional development including utilizing supervision and training. ▪ Will serve as a liaison and ambassador for the agency in the community. ▪ Will participate in and provide foster parent training or support of training as needed. ▪ Will attend case related conferences and meetings. ▪ Will provide case related documentation. ▪ Assess all youth 8 and older in Ansel Casey Life Skills Assessment and create IL plan per outcome and provide outcome to clinical worker as needed. ▪ Monitor foster parent training and complete annual re-certification process as directed. ▪ Gather data and complete the exceptional rate form in consultation with the clinical worker when acting on behalf of foster care.
▪ Maintains small child Therapeutic Crisis Intervention Certification, First Aid/CPR Certification, Community Safety Training, Casey Ansel Life Skills and family visitation as provided by Agency. ▪ Attends all mandatory in-service training and regularly attends supervision meetings. ▪ All related duties as assigned by immediate supervisor. SKILL, QUALIFICATIONS, KNOWLEDGE, ABILITY & PHYSICAL REQUIREMENTS: Minimum Education and/or Professional Qualifications/Skills ▪ Bachelor’s Degree in Human Services related field, with two years experience; or ▪ Master’s Degree in Human Services related field, no experience; or ▪ Associates Degree with three to four years experience. ▪ Must have a valid NYS driver’s license and adequate auto insurance. ▪ Must have a valid driver’s license, adequate auto insurance and meet the criteria for driving set forth in the New Directions Driver’s License policy and Auto Insurance policy. ▪ Ability to pass annual physical for Category 2 job classification in accordance with New Directions Employee Health policy.
