RN Care Transition Manager / RN Case Manager / PRN

Work location: Texas Health Arlington Memorial, 800 W. Randol Mill Road, Arlington TX 76012

Work hours: PRN - Weekday hours as needed.

Here’s What You Need

  • Bachelor's Degree, Bachelor of Science in Nursing is required. (CLIAFE) Must comply with CLIA regulations regarding foreign education equivalency required.

  • 3 years of RN experience in an acute care hospital is required.

  • 1 year of discharge planning and/or care management is highly preferred.

  • Ability to work independently in this role is highly preferred.

  • Must hold an active RN license - Registered Nurse Upon Hire is required.

  • CPR – Cardiopulmonary Resuscitation is required upon hire.

  • ACM – Accredited Case Manager upon hire is preferred OR CCM – Commission for Case Manager Certification or ACM – American Case Management Certification upon hire is preferred.

What You Will Do

Responsible for ensuring patients are transitioned to appropriate levels of care in a timely and effective manner. In addition to the required qualifications, a successful RN will:

  • Reviews the Texas Health Readmission Indicator List (THRIL) scores daily for all assigned patients and collaborates with the interdisciplinary team to identify high risk patients whose THRIL score may not have indicated appropriately.

  • Promotes discussion and assists in the identification of a primary care physician (PCP) for patients without a PCP
