Hiring for RN Case Manager
Job Description
Responsible for providing discharge planning and care management services for acute care patients. Identifies, facilitates, and advocates appropriate organizational and community resources to meet the plan of care and ensures that they are implemented in a cost-effective, efficient, and timely manner. Ensures consistent and reliable documentation of case management activities in compliance with all organization and department standards. Effectively manages and coordinates assigned caseload consistent with established criteria. May float to other locations.

This is a fully on-site position in Arlington, Virginia.
Work Environment
Inpatient Continuing Care at VHC 8:30-5:00; Every other weekend. Holiday commitment.
