Supportive housing - affordable housing paired with services designed to help people maintain their homes for the long-term - is widely recognized as a proven and cost-effective solution to chronic homelessness.
But we didn't stop at providing housing. Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.
Aging Services provides milieu-based services to tenants across our permanent housing portfolio focused on building community. Through time-limited, goal-directed case management services for those aging in place, the Aging Services team supports tenants in maintaining housing and improving their overall health and well-being.
Reporting to the Clinical Coordinator of Living in Place , the Aging Services Specialist , provides support to seniors throughout Breaking Ground and in using a Critical Time Intervention (CTI)-informed model; providing time-limited services focused on housing retention, resource development, and community integration intended to support older adults with aging safely in the community, and provides information and referral services; benefits application and navigation assistance; resource development support; and community integration programming. The case management for Aging Services is typically a 3– 6 month engagement and on a referral basis.
This position maintains a maximum caseload of 5 0 clients . The Aging Services Specialist serves as a member of the Tenant 360 team.
This position has a 35-hour work week. This is a fully in-person position . T he position may require occasional evening or weekend hours to support tenant events.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Carry a caseload of 40-50 participants for auxiliary, short- to medium-term, solutions-focused services using a Critical Time Intervention (CTI)-informed model
Conduct structured assessments to evaluate service needs and develop appropriate person-centered service plan based on assessment
Outreach potential Community partners as well as Breaking Ground programs to obtain referrals and build/ maintain caseload
Provide accurate and appropriate information on benefits, entitlements, programs, and case assistance and assist in accessing services, including referral, counseling participants and advocating and advocating on behalf of participants.
Arrange for provision of services needed by participants
Maintain complete and accurate case records by documenting engagements, attempted engagements, referrals, and collateral contacts timely in electronic case management record or through other means Prepare reports and other documentation related to service provision and disseminate information when necessary
Escort participants to appointments as needed or appropriate
Collaborate with other Aging Services Team members to implement group programming, including potential co-facilitation of socialization or similar groups
Conduct home visits as necessary to conduct assessments and/or engagements
Collaborate with external partners and site-based social services teams, including attendance at case review and case conferences, to ensure coordinated care for participants.
Provide supportive counseling
Perform other related duties as assigned
MINIMUM QUALIFICATIONS:
Bachelor’s degree or equivalent experience
1-2 years of experience working with older adults and/or populations who have experienced homelessness
Housing-based case management experience strongly preferred
Experience using electronic case management or health records preferred
Proficiency in solutions-focused interventions and motivational interviewing techniques preferred Superb collaborative skills
Creativity, strong problem-solving skills
Demonstrated success in working in a fast-paced environment
Ability to communicate (verbally and written) with diverse populations and stakeholders
Ability to travel throughout the 5 boroughs via public transportation or vehicle
Valid NY , NJ , CT, or PA driver’s license with a good driving record, as defined by Breaking Ground is preferred
Bi-lingual Chinese, Korean, Russian or Spanish preferred
