Developmental Pathways Case Manager - Active And Coverage in Aurora, Colorado

Developmental Pathways (DP) has an opportunity for a Case Manager - Active and Coverage in our Case Management departments. This role will be responsible for managing caseloads in coverage to ensure supports are provided and will complete any coverage tasks including required documentation related to monitoring and paperwork support as needed such as revisions, CAFs, DocuSign, etc. This role will also work with the Program Manager to determine missed monitoring amongst coverage and will assist in completing warm-hand off's as it relates to coverage.

This is a 40-hour per week, hybrid position, with the benefit of working remotely and in-person coming into our headquarters office in Aurora, CO and/or off-site locations.

Requirements

Bachelor's Degree; or Five (5) years of experience in the field of Long-Term Services and Supports; or (LTSS), which includes Developmental Disabilities; or Some combination of education and relevant experience appropriate to the requirements of the position Valid Colorado motor vehicle license and provide proof of motor vehicle insurance Meet our driving requirements - no suspensions and no more than two (2) tickets in the past three (3) years

Upon request, qualifying need, and approval, driving requirements may be waived in lieu of reliable transportation.

Preferred

One (1) year of experience in developmental disabilities/long-term services and supports

Additional Responsibilities

Responsible for verifying eligibility and developing, monitoring, and assuring implementation of the Service Plan (SP) Ensures effectiveness of services delivered (as authorized by the service plan) and ensures identified outcomes are met, advocates for individual service needs Provides supports with coverage email management, research, tracking, assignments, etc. Serves as subject matter expert for applicable programs and services; serves as resource/point of contact for complex cases Ensures services and supports identified in the annual service plan are coordinated Provides care coordination services and interventions by referring, educating, negotiating, and mediating with the individual and external providers/agencies Completes accurate case documentation in appropriate tracking and databases; prioritizes workload and meets all required timelines Completes required documentation related to monitoring activities and other targeted case management activities as PMPM; evaluates need and effectiveness of services requested and delivered Ensures the accuracy and timely entry and distribution of required documentation to all necessary team members, including, but not limited to providers, individuals, and family members Maintains knowledge of state/ federal regulations, policies and procedures regarding long-term services and supports, Medicaid waiver and state funded programs and other community resources Maintains a professional and ethical manner in all interactions and meets performance quality, customer service, and coordination standards as assigned by department leadership; maintains appropriate boundaries
