Job Overview

The Clinical Specialty Coordinator is a Registered Nurse (RN) who coordinates and oversees a program specific in a defined specialty population. The Clinical Specialty Coordinator works across the continuum to develop, implement, and support quality improvement efforts that improve outcomes for the specialty practice.Areas of focus include program development, standardization of practice and documentation, communication, sharing of information, hand-offs, transitions, advocacy, patient and family guidance, education, resources, and specialty specific expertise. The Clinical Specialty Coordinator provides guidance and support to staff in the management, provision and coordination of care in their area of expertise.

Qualifications

Education:Bachelor of Science in Nursing (BSN) Experience:Four years of nursing/respiratory care experience Certification(s): BLS/CPR from the American Heart Association with at least 6 months left before expiration is required upon hire. Achievement and maintenance of certification in one of the two categories: Successful completion of required internal training within six (6) months of hire or transition into role Licensure(s):Registered Nurse (RN) OR Registered Respiratory Therapist (RRT) licensure in the state of Colorado is required. Professional Membership(s):Active membership in a professional nursing or healthcare organization is required.

Coordinates program specific care by: assessing patient/family needs while obtaining treatment in spec fic program establishing and implementing program specific Care Plan in partnership with the patient/family and multi-disciplinary team to proactively support the patient/family connecting families back to primary care provider and Clinical Care Coordinator, if applicable documenting within the standardized tools and maintaining the Care Plan evaluating patient/family needs and outcomes to continuously update the Care
