Completes bio-psychosocial evaluations, treatment plans, and provides brief, evidence-based psychotherapy through a series of face-to-face sessions (virtual and in-person). Maintains written documentation and generates charge capture for all psychotherapy services rendered, including the initial assessment and follow-up visits consistent with organizational standards.Supports patients/caregivers by (a) sharing information and perspectives on how other life factors are affecting chronic illness or other medical condition and (b) helping patients/families to build coping strategies, psychological skills, and ways to improve your daily function and quality of life. Provides care management to include connecting patients to needed resources such as prescription assistance programs and other resources in the community.Participates in value-based research initiatives and research for patient care operational improvements. Participates in screening, tracking, and reporting of patient outcomes and social work interventions.Provides discipline-specific knowledge to interdisciplinary team around mental health, social determinants of health, and innovative ways to approach chronic illness in patients. Participates in department meetings, continuing education opportunities, academic and research opportunities, and supervise masters level interns to optimize patient best practice and interdepartmental collaboration.Develops and maintains working relationships with behavioral health providers in the community to create a network of aligned providers. Maintains a highly collaborative approach to a interdisciplinary and inclusive cohort of clinical team members, as well as the local, regional and national professional communities.Marginal/Incidental FunctionsOther related functions as assigned.
