Foundation Communities Permanent Supportive Housing Navigator in Austin, Texas

Position Description: The permanent supportive housing navigator provides housing navigation to adult individuals entering permanent supportive housing (PSH) and provides basic case management services to applicants and residents. The main responsibility for this position is helping applicants to obtain and prepare necessary documentation for housing applications. This position also provides basic case management to residents to assist them in successfully maintaining housing, accessing mental and primary health care, financial management, education, and employment services.

Primary Duties/ Responsibilities

  • Conduct in-person navigation services for individuals identified through Coordinated Assessment to include working within the homelessness response system to locate individuals and help them to complete required paperwork for tenancy. This work will take place within the Austin community, frequently outdoors and at other community agencies.

  • Collaborate with individuals to complete Foundation Communities and Housing Authority of the City of Austin housing applications. Gather necessary documentation, fill out applications, and assist applicants to attend required appointments to complete the housing application process.

  • Conduct basic case management to increase client self-sufficiency through an interdisciplinary team approach. Work closely with area providers, partner agencies and the larger Resident Services team and to increase residents’ housing retention, income stability, health and wellness, and social connectedness.

  • Complete applications for benefits assistance with residents (SNAP, Medicaid, Medicare Savings Plans, etc.) and assist in gathering necessary documentation for SSI/SSDI Outreach, Access, and Recovery (SOAR) applications.

  • Effectively assess and diffuse crisis situations by using de- escalation techniques, calling proper authorities in the event of an emergency, and thoroughly communicating and documenting incidents and outcomes as appropriate.

  • Develop creative strategies in alignment with department leaders to build relationships and rapport with   residents, while providing assertive outreach to determine areas of need, increase residents’ participation in their treatment, and connect residents to appropriate resources. Develop and hold group and provide one on one coaching that assists with residents developing basic life skills and good tenancy skills.

  • Collaborate with Property Management following a blended management model. Meet routinely with Property Management staff to facilitate quickly approved applications and renewals and recertification of leases.

  • Document delivered services in accordance with grant requirements and FC documentation systems.

    Minimum Requirements

    • At least one year of experience working directly with adults experiencing homelessness, exiting homelessness, or at risk of  homelessness.
    • At least one year of experience working directly with adult populations living with mental health and substance use challenges.
  • Bachelor’s degree in social work or related field or 2 years professional experience in closely related field.

  • Ability to work independently, problem solve, think critically and independently and effectively communicate with people from diverse backgrounds.

  • Must have strong interpersonal skills, good organizational methods and be skillful at multitasking. Able to proactively form warm, professional relationships with residents by demonstrating respect, kindness, and availability, as well as a person-centered, trauma-informed, strengths-based orientation in all work.

  • At least one year of experience doing outreach or housing navigation with individuals. One year of experience working in supportive housing may serve in lieu of outreach or navigation experience.

    Preferred Requirements

    • Proficiency in Spanish or ASL.

    Working Conditions

    The work conditions are a balanced mix of in-office setting, and out of doors work throughout the Austin community, and in other area agencies that are part of the homelessness response system. The noise level in the work environment is usually moderate to loud. An occupational exposure to blood, or other potentially infectious materials can be expected. Must be able to use computer and cell phone extensively.

    Travel: This position requires daily travel between locations and will require use of a personal vehicle. Travel is reimbursed at the approved IRS mileage rate.

    Physical Requirements

    While performing the duties of this job, the employee is both outdoors and an in-office, setting and is regularly required to see, talk and hear. The employee frequently is required to stand, walk, and sit. The employee is frequently required to use hands. The employee must occasionally lift and/or move up to 10 pounds. Specific vision abilities required by this job include close vision and the ability to adjust focus.

    Compensation

    $25/hour
