STAFF NURSE (RN or LPN)

**[FUNCTION: [To provide skilled nursing care to hospice clients as assigned by Nurse Manager/Nurse Supervisor and to cover after-hours and weekend nursing on a rotation basis with other Staff Nurses and Nurse Manager/Nurse Supervisor.

1. Active license in good standing to practice nursing in North Carolina.
2. Ability to work as part of the hospice treatment team.
3. Skilled in assessment, care planning, patient teaching, treatments appropriate to hospice clients, documentation and communication.
4. Knowledge of the dying process and stages of grief.

**[SUPERVISION: [The Staff Nurse is supervised by the Nurse Manager/Nurse Supervisor and works in concert with the Interdisciplinary Team (IDT).

[RESPONSIBILITIES:]{new="" roman="" times=""}]{new="" roman="" times=""}]{new="" roman="" times=""}**

1. Assess patient and family needs and provide teaching as needed.
2. Provide treatments requiring nursing skills
3. Update the Plan of Care as needed, based on assessment, with appropriate interventions and expected outcomes.
4. Coordinate services and care planning with the Nurse Manager/Nurse Supervisor and the Interdisciplinary Team (IDT)
5. Communicate significant changes in patient conditions and/or needs to the Nurse Manager/Nurse Supervisor and the Attending Physician.
6. Obtain and implement physician orders in a timely and appropriate manner.
7. Attend and participate in IDT meetings
8. Document patient and family contacts in a legible, comprehensive, confidential and timely manner.
9. Participate in weekend on-call rotation.
