Glade Run Lutheran Services Family Based Case Manager in Beaver Falls, Pennsylvania

Family Based Case Manager Position Summary: Family Based Mental Health Services provides intensive family focused therapy, delivered primarily in the home, to families who have a child with a diagnosed mental health condition and are at risk for out-of-home placement. The service is team delivered by two staff who focus on the mental health needs of the child, the relationships within the family system, and the natural strengths of the family.

    QUALIFICATIONS : CASE MANAGER LEVEL 1

  • Bachelor’s Degree in any field AND

  • At least Twelve (12) college level semester hours in humanities or social services AND

  • (1) One year of experience in a CASSP system program AND

  • Be enrolled for certification by the Office of Mental Health and Substance Abuse Services (OMHSAS) as a mental health family based worker. *This qualification is an expectation upon hire into the role.

CASE MANAGER LEVEL 2

  • Bachelor’s Degree in psychology, sociology, social work, nursing, rehabilitation, pre-med, theology, or anthropology plus one (1) year of experience in a CASSP system program; OR

  • Be a licensed RN plus 1 year of experience in a CASSP system program and be enrolled for certification by OMHSAS as a mental health family based worker;

  • AND be enrolled for certification by the Office of Mental Health and Substance Abuse Services (OMHSAS) as a mental health family based worker. *This qualification is an expectation upon hire into the role.

REQUIREMENTS:

  • Must have audio/visual/verbal ability to perform essential job functions.

  • Must complete and pass agency Therapeutic Crisis Intervention training.

  • Must have emotional stability to perform essential job functions.

  • Must possess excellent verbal and written communication skills.

  • Valid driver's license, acceptable driving record and access to reliable transportation are required.

  • Ability to adapt hours to needs of position. Will need to work evenings regularly and weekend hours as needed.

  • Utilize and manage agency issued phone during working hours

  • Ability to remain organized despite pressured situations.

  • Must be able to work independently and display time management skills

  • Eligible for PA State Police clearance, PA Child Abuse History clearance and FBI clearance.

  • Must be able to identify with the agency Mission Statement and support the agency’s Core Values.

  • May be required to provide back-up staffing for on-call rotations (Case Manager Level 2 Only)

AREAS OF EXPECTED GROWTH AND DEVELOPMENT (Level 1)/KNOWLEDGE AREAS (Level 2):

  • Principles of Ecosystemic Structural Family Therapy

  • Connecting clients to community resources and empowering clients to access resources independently

  • Collaboration with other providers in the community and building professional relationships outside of the agency

  • Crisis intervention

  • Familiarity with Child Protective Services and mandated reporting

  • Mental health diagnosis and effective interventions

  • Mental health services in the community

  • Familiarity with psychological and psychiatric evaluations

  • Creating treatment plans with goals appropriate to the ESFT model and writing DAP progress notes

  • Familiarity with the juvenile justice system

  • Collaborative documentation

  • Process/procedure related to IEP’s

POPULATION SERVED:

  • Children and adolescents, who are at risk or have mental, emotional, and/or behavioral health needs which are causing distress and difficulty functioning in their home, school, and/or social environment and who meet criteria for a mental health diagnosis through the Diagnostic and Statistical Manual, fifth edition (DSM5).

  • The child/adolescent 14 years and older consents to treatment and/or has a legal guardian who consents to treatment.

  • Children, adolescents, and adults being stepped down/discharged from a more intensive service such as Acute Partial Hospitalization program, inpatient psychiatric hospitalization, or residential treatment facility.

  • Children, adolescents, and adults currently in Intensive Behavior Health Services (IBHS), Outpatient Mental Health Services, and Acute Partial Hospitalization programs and require a more intensive service

CORE COMPETENCIES:

  • Complete all required client paperwork within 30 days of admission.

  • Will work collaboratively with families in order to provide support and education related to their individualized needs.

  • Will work within the Ecosystemic Structural Family Therapy framework.

  • Will work with an assigned Mental Health Professional (partner).

  • Will participate in agency and state mandated trainings.

  • Will maintain compliance with state mandated supervision requirements
