Outreach Development Corporation Case Manager in Bellport, New York

We are seeking an experienced and extremely motivated FT SOR Case Manager to work in our outpatient program located at 11 Farber Drive, Unit D, Bellport, NY. Qualifications:

  • CASAC-T, CASAC, LMSW, LMFT, LMHC preferred.

  • Will consider CASAC-P eligible with relevant substance abuse treatment and case management experience.

  • Experience working in an outpatient 822 program.

  • Experience working with clients with substance use/mental health disorders.

  • Experience providing case management and crisis intervention services required.

  • Experience providing group counseling services.

  • Experience providing advocacy for clients.

  • Commitment to providing quality services and outcome driven performance measures.

  • Action-oriented, adaptable, and innovative approach to program planning.

  • Ability to work effectively in collaboration with diverse groups of people.

  • Experience with AVATAR electronic health records a plus.

  • Clean and valid NYS driver's license required.

The primary duties and responsibilities include, but are not limited to:

  • Case management and crisis intervention to assist in the coordination of treatment services in line with workforce initiatives and promote access to transitional support services and clients dealing with sanctions, barriers to employment and self-sufficiency.

  • Facilitation of support group such as life skills.

  • Advocates on behalf of clients to obtain needed services.

  • Interfacing with appropriate agencies and systems (DSS, probation, courts, etc.) on behalf of the client and assisting clients in meeting all mandates.

  • Ongoing case management related to housing, transportation, childcare, food and other related needs.

  • Conducts toxicology tests, as needed.
