Position Overview:
Provides psychosocial assessment, individual/family therapy, discharge and aftercare planning to patients and their families in an outpatient setting. Delivers services and treatments based on recovery-oriented services, family systems, and evidenced based treatments. Provides consultation and training on psychosocial aspects of human behavior and complex environments. Serves as the primary contact for involving and educating patients, families, and significant others in the process of treatment and mental health recovery. Acts as a liaison to community programs and develop strategic partnerships to enhance aftercare plans of patients and their families. This job is performed in both inpatient and outpatient as well as in an individual or a group setting.

JOB ACCOUNTABILITIES:
Performs the following duties under supervision of a Licensed Clinical Social Worker:
Gathers appropriate data and formulates relevant assessment of patient and family’s medical, social, and emotional situations using effective interviewing techniques, consultation with healthcare team and chart review.
Establishes and periodically revises the treatment plan on the basis of comprehensive and ongoing psychosocial assessment of patient/family's concerns, strengths, needs, clinical impressions, and issues specific to spiritual and cultural values.
Develops personalized treatment plan to delineate strengths, deficits and high-risk psychosocial needs.
Provides educational and therapeutic groups. Documents patient goals and response to group on therapeutic activity.
Attend treatment-planning meetings to give and receive feedback. Discuss progress/resistance of patient. Document updates on treatment plan.
Identifies and effectively deals with resistance and conflict in work with clients (including patients, families, healthcare team members and community representatives). Appropriately intervenes to decrease tension and assist clients in evaluating non-productive behaviors, thought processes and coping mechanisms.
Compiles and maintains accurate, current, legible patient records using approved forms and format. Documents all assessments, plans, interventions, patient/family involvement, outside agency communication, interdisciplinary contacts and case closure according to social work and patient care area standards.
Demonstrates understanding of the most recent edition of DSM diagnostic criteria for disorders in children and adolescents as well as an understanding of human growth and development and identifies age-related developmental tasks.
Applies the principles of behavior modification, assertiveness training and supportive therapy, and their application. "
COMPLIANCE:
• Maintains strictest confidence of all patients protected health information (PHI) and protects all PHI from accidental, intentional, or inappropriate disclosure.
• Performs care in compliance with department standards, policies, and procedures, as well as local, state and federal regulatory agencies to deliver the highest level of patient privacy and care.
• Retains certification and completes mandated continuing education requirements to meet facility and regulatory agencies accreditation and licensure requirements.
EDUCATION:
Master's: social work, counseling, or related field.
CERTIFICATION & LICENSURE:
LCSW-Licensed Clinical Social Worker
OR LMFT-Licensed Marriage & Family Therapist
TYPICAL EXPERIENCE:
2 years of recent relevant experience.
SKILLS AND KNOWLEDGE:
Knowledge of various principles, procedures, views, and equipment used in the treatment of patients with varying disorders.
Ability to interpret a variety of data and instructions, furnished in written, oral, diagram, or schedule form.
Demonstrates working knowledge of available specialized treatment centers including mental health and substance abuse services and facilities.
Possess written and verbal communications skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical people.
Well-developed time management and organizational skills, including the ability to prioritize assignments and work within standardized operating procedures and scientific methods to achieve objectives and meet deadline.
General knowledge of computer applications, such as Microsoft Office Suite (Word, Excel and Outlook), electronic health records (EHR).

Work independently, as well as be part of the team, including accomplishing multiple tasks in an environment with interruptions.

Make timely referrals for specialized evaluations, programs, and services.

Pay Range is $53.74 to $72.27 / hour
The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
