Position Summary:
We have an exciting opportunity to join our team as a Physical Therapist, Per Visit - Home Health Care - Eastern Long Island. In this role, the successful candidate provides safe therapeutic age appropriate physical therapy to promote the improvement of functional mobility and physical ability and quality of life. Uses clinical expertise to plan the delivery of care through examination evaluation planning implementation and re assessment
Job Responsibilities:
- Performs an objective functional evaluation and re assessment of patient including the use of appropriate tests and measures; patient safety issues and home environment factors; and establishes a plan of care with measurable and functional short term/long term goals
- Performs other duties as assigned
- Assists Agency leadership with achieving corporate goals improving quality of patient care outcomes and patient satisfaction Participates in Agency quality assessment/performance improvement initiatives
- Completes and maintains accurate complete and timely documentation reflecting the assessment examination diagnosis prognosis plan of care education intervention and evaluation according to Federal Department of Health DOH and The Joint Commission TJC requirements
- Re assesses patient progress toward short term/long term goals and modifies the plan of care accordingly
- Educates patient and family regarding patient s functional status and plan of care Provides a home exercise program and instruct on home safety factors
- Interacts and collaborates with other members of the healthcare team to optimize patient outcomes and health care goals
- Implements and modifies appropriate interventions/treatments as indicated based on the patient's functional status diagnosis age patient response and status in conjunction with the plan of care
Minimum Qualifications:
To qualify you must have a Bachelor's Degree Physical Therapy NYS Registered or APTA Accredited Physical Therapy Program Current and valid NYS Physical Therapy license and registration 1- 2 years Hospital Inpatient or Orthopedic Outpatient or Pediatric setting Excellent communication both oral and written critical thinking and customer service skills Basic computer skills
Preferred Qualifications:
Master's Degree Physical Therapy NYS Registered or APTA Accredited Physical Therapy Program Post Doctoral Doctorate in Physical Therapy DPT Physical Therapy NYS Registered or APTA Accredited Physical Therapy Program, 1 - 2 years Home Care experience

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $65.000000 Hourly. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
