Knife River Care Center Physical Therapist (PT) in Beulah, North Dakota

Performs evaluations and develops effective Physical Therapy treatment plans to restore, maintain, or prevent decline of resident function, by planning and administering medically prescribed therapy treatments in accordance with federal, state, and professional standards. The Physical Therapist must also be able to set realistic and achievable goals for the residents as well as document and record the residents conditions using the correct documentation. Essential Functions, Roles, and Responsibilities

  • Administers professional services within scope of practice for PT in a professional and safe manner.
  • Meets the residents goals and needs and provides quality care by assessing and interpreting evaluations and test results; determining physical therapy treatment plans in consultation with physicians or by prescription.
  • Responsible for the assessment of residents that require rehabilitation services which includes; providing direct resident care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
  • Screen each new admission regarding their need for physical therapy and restorative nursing services. Write suggestions on treatment and progress sheets for physician's approval.
  • Write a treatment plan for each resident receiving physical therapy services and accurately document services provided.
  • Evaluates effects of physical therapy treatments by observing, charting, and evaluating the residents progress; recommending adjustments and modifications.
  • Establish and administer a treatment program with specific goals determined according to the residents capacity and tolerance under the direction of the Physician.
  • Demonstrate knowledge and proficiency of all rehabilitation services modalities.
  • Establish, review and update care plans and MDS as required.
  • Communicate to nurse managers/ charge nurse any physical therapy and restorative nursing service needs or achievements of the residents that should be brought to the attention of the physician.
  • Instruct therapy aide in procedures to follow in carrying out any new restorative nursing services.
  • Complies with federal and state legal and professional requirements by studying existing and new regulations.
