Position Summary:

The Resident Care Coordinator performs a variety of duties to support the Resident Care Director to ensure that the department runs smoothly. Answers and directs calls, takes messages, meeting and greeting families and residents. Monitors and records resident usage of supplies and equipment utilized within the department. Responsible for coordinating the scheduling of resident care and liaising with outside agencies to cover open shifts.

Education/Experience:

Ability to provide assistance to resident in a professional and caring manner is required. Skill and knowledge is required in English grammar, writing, spelling and communication. Requires organization skills and the ability to prioritize and handle multiple tasks. File maintenance skills are essential. Requires the ability to respond calmly and competently to emergency situations. High school graduate or GED is required.

None are required for this position.

Essential Job Functions:

The employee must be able to perform these essential functions with or without reasonable accommodation by The Herrick House without posing a direct threat to himself, herself or others.

Coordinator Responsibilities:

  1. Answers Wellness Center telephone when nurse is not present.

  2. Establishes resident shower, incontinent pail and bed linen change schedule.

  3. Files all correspondence for the nursing and resident care departments.

  4. Prepares ancillary billing for the department monthly by collating meal slips, and medical charge slips.

  5. Assists in coordinating orientation of new staff members, reviewing schedule, colleague connection etc.

  6. Fills in to do resident care when staff is short-handed as appropriate.

  7. Checks that weights have been done and recorded daily.

  8. Tracks and maintains resident care ancillary supplies.

  9. Covers reception desk as required.

  10. Delivers resident mail and daily newspapers.

  11. Accompanies pharmacy deliveries to residents’ apartments as requested.

  12. Develops and maintain Resident Care Department schedule with DRC.

  13. Tests E-Call system as required.

  14. Tracks incidents for QA and maintains binder of Incidents.

  15. Orders incontinent briefs weekly.

  16. Assists the Director in data collection and correction for Performance Improvement activities. Takes minutes for QA meetings.

  17. Overseas the disposition of all resident medical records.

  18. Assists nurse with preparing resident for emergency transport.

  19. Updates Kronos (employee work time system) daily and as needed, and assists with payroll biweekly.

  20. Creates daily assignment sheets and assisting in filing of daily assignment sheets as well as daily logs.

  21. Updates emergency contact list for emergency preparedness.

  22. Print Lifeline reports daily, capturing a 24 hour period.

Care of Resident – RCC Clinical Oversight/Performance:

  1. Assist with in-house moves.

  2. Reports and changes in resident’s condition to the Wellness Nurse.

  3. Responds to resident’s requests/needs promptly.

  4. Records resident care staff attendance and tardiness.

  5. Performs similar and other related duties.

Care of Resident – RCC Housekeeping Tasks

  1. Demonstrates proper care of cleaning parker tubs.

  2. Checks on residents’ apartment when resident is admitted to hospital (clears perishables).

  3. Track ancillary charges.

  4. Orders supplies for resident care department and maintains inventory and oversight of storage supplies.

  5. Assist with moving resident’s personal belongings from one apartment to another as required.

Care of Resident – RCC Meal Service Functions:

  1. Delivers meal trays to resident apartments.

  2. Retrieves trays within 2 hours of delivery.

  3. Transports residents in a timely fashion to and from meals and activities.

  4. Track meal charges.
