Brockton Hospital Clinical Social Worker in Brockton, Massachusetts

Signature Healthcare’s Patient Centered Medical Home is hiring for a Full Time Social Worker in Brockton, MA.

The clinical social worker, as part of the Patient Centered Medical Home team in the Signature Medical Group practice, engages in a collaborative process with primary care providers which assesses, plans, implements, coordinates and monitors options and services to meet a patient’s whole healthcare needs. The social worker assists in the identification of appropriate behavioral health care and other community resources ensuring that available resources are being used in a timely and cost effective manner in order to obtain improved patient outcomes and an improved patient experience. The qualified candidate will have experience in a medical setting as well as expertise in evidence based practices relevant to working with patients and their families with co-occurring mental health and substance use disorders. The social worker will demonstrate cultural competence in care delivery. Familiarity with Patient Centered Medical Home is a plus. The social worker will report to the Manager of Behavioral Health Services.

Clinical Social Worker Roles and Responsibilities:

  • Addresses the total patient inclusive of medical, psychosocial, behavioral, cultural and spiritual needs.

  • Collaborates with the primary care providers and other members of the multidisciplinary team to establish measurable healthcare goals and prioritization of the patient’s needs as well as identifying any barriers to care.

  • Identifies and evaluates patients with mental health and substance use disorders in order to provide appropriate care. Works with community behavioral health partners to facilitate patient access to care.

  • Responds to patients in crisis when needed.

  • Monitors behavioral health care provided to SMG patients in the community.

  • Conducts Telehealth visits for patients seen in ED for BH diagnoses.

  • Conducts Telehealth visits for patients discharged from Inpatient BH Admissions.

  • Tracks relevant screenings and registries via the electronic medical record.

  • Consults with primary care providers around the impact of behavioral health disorders on chronic medical conditions and plans how to address these issues.

  • Meets with patients and/or family members for short term therapy when necessary.

  • Uses evidence-based practices in treatment such as solution focused short term therapy, motivational interviewing and strength based approaches in treating patients.

  • Refers to and collaborates with the Community Health Worker to locate concrete services for patients.

  • Attends team, departmental and practice meetings.

  • Pursues ongoing education to maintain competence in practice.

  • Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.

    Required

  • Master of Social Work degree or Masters in Mental Health Counseling degree from an accredited graduate program

  • Certification/Licensure: LICSW, LCSW or LMHC

  • Experience: 3-5 years in a medical setting required

  • Comfortable with medical providers and medical terminology

  • Excellent interpersonal, verbal and written communication skills

  • Knowledge of crisis intervention and evidence based treatment techniques

    Preferred

  • Knowledge of the patient centered medical home preferred
