Be a Leader in Launching a Direct Care program for a Supportive Housing Community

Are you excited about the opportunity to build something new? We’re seeking a Manager of Supportive Housing Programs to help launch of a new initiative, overseeing a Group Adult Foster Care (GAFC) program. This is a unique opportunity to help shape a program from the ground up and hire a dedicated team of direct care staff who will make a lasting impact in the lives of vulnerable individuals.Whether you come from a background in case management, social work, or have experience working with individuals with disabilities, mental health challenges, or other vulnerable populations, this role is a perfect opportunity to grow your leadership skills while making a difference in the community.What You’ll Do:

As the Manager of this new program, you’ll be responsible for overseeing day-to-day operations, ensuring compliance with state regulations, and building strong partnerships with external agencies. In addition, you’ll play a key role in hiring and developing a team of compassionate direct care staff who will work directly with clients providing care management.

Key Responsibilities:

  • Conduct on-site case management visits as required by GAFC regulations, including obtaining social history and conducting a psychosocial evaluation for each client.

  • Track and keep care-management progress notes for each client visit.

  • Provide timely response and assistance and response to urgent client needs.

  • Collaborate on a team to develop and update client care plans.

  • Build strong relationships with GAFC clients and partnering agencies to ensure their needs are met according to state guidelines.

  • Lead the hiring process for your team of direct care staff, ensuring the right individuals are selected and onboarded for success.

  • Provide supervision, guidance, and training for your team, ensuring they have the tools and knowledge to provide exceptional care.

  • Oversee staff scheduling to ensure fair and adequate coverage, while supporting the direct care team’s growth and development.

  • Educate staff and clients on care techniques, medication safety, and available resources.

  • Lead regular staff meetings to communicate program changes and ensure compliance with all guidelines.

What We're Looking For:

  • Education: Bachelor Degree required

  • Experience: 1-2 years of supervisory experience in a related field.

  • At least 2 years of experience in case management, social work, or working with individuals with disabilities, mental health challenges, or substance use disorders.

  • Strong leadership, communication, and problem-solving skills, with the ability to inspire and support a team.

  • Strong commitment to supporting clients and team members within the GAFC program.
