Bowery Residents Committee Entitlements Discharge Planner - 367 East 154th Street in Bronx, New York

DUTIES/RESPONSIBILITIES:

Develop orientation procedure for incoming residents. Meet with prospective candidates for housing center with intake coordinator regarding admission/ discharge. Procure entitlements. Teach clients and staff application procedures. Make referrals to government agencies and escort clients as needed. Reconcile all client beneficiary accounts. Prepare and monitor all client accounts and keep appropriate records. Responsible for completing forms required by the agency. Review discharge/ referral packets. Responsible for distribution of Food Stamps. Related duties as assigned.

HOURS:

Full-time 35 hours per week

  • Tues-Fri 8:15-6pm

  • One day a week at Cecil Ivory House Program (149 West 132nd Street)

QUALIFICATIONS:

High School diploma, BA preferred. Entitlement experience required (Welfare, Medicaid, Veterans, etc.) 2 years sobriety if recovering.

  • Vaccination preferred but not required.

PROGRAM DESCRIPTION:

The Metropolitan Apartment Program (MAP) is a transitional housing program targeting formerly homeless clients who are either mentally ill or dually diagnosed as mentally ill and chemically addicted. Funded by the NYS Office of Mental Health and Medicaid, MAP comprises apartments in Manhattan and the Bronx for 87 clients who can function and thrive in a more independent living environment than a traditional community residence. The mission of MAP is to provide a safe and supportive environment where residents partake of rehabilitation interventions that will assist them in the attainment of their work, social and community living goals. A variety of services are available, including case management, substance abuse groups, money management, daily skills training and recreational activities.
