Amergis RN Hospice - Case Manager in Brookfield, Wisconsin
The Registered Nurse - Hospice is responsible for providingcare, comfort and support to terminally ill patients. The Registered Nurse - Hospice assumes theresponsibility and accountability for the delivery of hospice patient care invarious settings. The Registered Nursedemonstrates the ability to make clinical judgments in an effective andefficient manner with supervision. TheRegistered Nurse will also demonstrate critical thinking and performanceability in the coordination of patient care. Consistently performs according to nursing standards and is accountablein managing patient care and assisting others in the management of patientcare.
Essential Duties and Responsibilities:
Utilizes the nursing process to assess, plan, implement andevaluate patient care
Performs focused interview to identify specific patientneeds
Assesses signs and symptoms indicating physiologic andpsychosocial changes in patient’s condition
Collects, analyzes, and interprets data and information fromhealth care members and documents actual and/or potential nursing diagnoses
Documents the patient’s plan of care using identifiednursing diagnoses, expected patient outcomes, and selected nursinginterventions
Identifies and prioritizes actions based upon patient carerequirements
Performs interventions according to identified priorities,plan of care, and the hospital policies and patient care outcome standard
Provides a safe, comfortable and therapeutic patientenvironment
Initiates and evaluates patient and/or family education andprovides guidance and support
Revises plan of care according to evaluation, changes inmedical plan of care, effective/ineffective nursing interventions
Documents the patient’s response to nursing and otherinterventions
Uses clinical judgment in evaluation activities to meetpatient care needs of an assigned floor/unit including establishing priorities
Monitors safety and effectiveness of the environment andequipment
Advises grieving family members on coping techniques andcommunity resources
Performs other duties as assigned/necessary
Minimum Requirements:
Current Registered Nurse license for the state in which thenurse practices.
Complies with all relevant professional standards ofpractice
Participation and completion of Amergis' Competency programwhen applicable
Current CPR if applicable
TB questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or stateregulation)
Must meet all federal, state and local requirements
Successful completion of new hire training as applicable tojob site
Understand patient confidentiality and HIPAArequirements
Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required
Computer proficiency required
Must be at least 18 years of age
