Description

You will be responsible for providing intensive care management to chronically ill patients and their families by advocating and navigating through the health care system. The Care Manager will promote linkage development and monitor the effectiveness of linkages with other providers. The care manager ensures the coordination of care on the client’s behalf promotes compliance with medical appointments, encourages client self-sufficiency, empowerment, and overall wellness.

Supervisory Responsibilities:

  • Provide care management, crisis intervention, referral, and care coordination services to the assigned caseload.

  • Collaborate and consult with clinicians and members of the client’s care team to develop and implement client care plans.

  • Ensures that all documentation and billing is completed and current.

  • Completing a comprehensive assessment or reassessment inclusive of medical, behavioral, rehabilitative, and long-term care and service needs.

  • Diligently advocates for the client to obtain the full range of needed services.

Essential Functions and Duties:

  • Conduct client outreach and engagement activities to assess ongoing need, promote continuity of care and improve health outcomes.

  • Ensure fact finding as necessary and the completion of intakes.

  • Collaborate with community-based providers to support utilization of services based on client need.

  • Facilitate and develop a client emergency/crisis intervention plan.

  • Perform other related duties as assigned.

Requirements

Required Skills/Abilities:

  • Solid organizational and follow up skills.

  • Strong communication skills.

  • Computer literate (Microsoft word, Excel, PDF, PPT, Teams, Zoom).

  • Excellent interpersonal skills.

  • Capacity to communicate clearly and directly with others, orally, electronically and written. Bilingual English/Spanish or French preferred.

    Education and Experience:

  • MSW/MPH/BA/BS in health and/or human services required and two-years qualifying experience in behavioral health and/or care coordination.

  • CANS (Children Assessment Needs and Strength) and HARP certified.

Physical Requirements:

  • Prolonged periods sitting at a desk and working on a computer.

  • Must be able to conduct field visits in all types of weather.

  • Flexibility to travel throughout the boroughs of NY

Salary Description

$55,000
