CASEWORKER II 50 (283) Open Until Filled Salary: $37,598 to $45,819 Annually Salary is based on education and experience

Job Description: As a LIDDA Service Coordinator, provide crisis prevention and management, monitoring, assessment, and service planning and coordination to children and adults with intellectual or developmental disabilities (IDD) enrolled in LIDDA General Revenue Services, Community First Choice, and Texas Home Living Medicaid Waiver Program; and residing in local service area of Brazos, Burleson, Grimes, Leon, Madison, Robertson, and Washington Counties. Service Coordination involves accessing medical, social, educational, and other appropriate services that will help an individual achieve a quality of life and community participation acceptable to the individual which is based on a Person Directed Plan (PDP). The Service Coordinator will negotiate and facilitate the array of services and support needed to address the individuals personal goals and desired outcomes as identified through the Service Coordination Assessment and Service Planning Team. The Service Coordinator is responsible for ensuring planned services and support are implemented and appropriately documented. Will advocate and ensure consistent interactions with other agencies. Education: Requires Bachelors or advanced degree with a major in a behavioral, social, or human service field, including but not limited to psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice. Experience: Prefer experience working with individuals with IDD. Prefer Qualified Intellectual Disabilities Professional (QIDP) required education and experience requirements; must be able to work independently as well as with a team. Good organizational skills are necessary. Hours of Work: 8:00am5:00pm Monday through Friday; may require after-hours flexibility to meet the needs of the person in service/program. Travel within seven county service area Licensure/Certifications: Must have reliable transportation and a safe driving record. Must have a valid Texas drivers license and be insurable by Agency insurance. Physical Requirements: Ability to drive a vehicle, ability to communicate effectively; knowledge of case management principles, objectives, standards and methods; knowledge of Medicaid waiver program services and non-Medicaid waiver program services;; skill in determining individual needs assessment and the ability to respond to changes required; ability to comprehend and implement program goals; ability to pass the Preventative Management of Aggressive Behavior (PMAB) Training as well as all other physical requirements for this position.
