As a Clinical Counselor at Horizon, you will…

  • Complete 4-hour SBIRT training course.

  • Work as part of the school counseling team.

  • Become familiar with use of CRAFFT 2.1+N, PHQ-A, and PHQ-9.

  • Become familiar with all consent and referral forms to be used with population of focus.

  • Obtain parental/legal guardian consent for screenings.

  • Screen adolescents and young adults (clinician-administered unless otherwise indicated) for substance use disorders (SUDs) using screenings exactly as they are without modifications.

  • Conduct Brief Interventions, per screening results, for adolescents and young adults.

  • Provide validated screening tools in the language of the student’s preference and enlist support from language interpretation services to communicate effectively with students and families when needed.

  • Work closely with families when an adolescent/young adult screens positive for SUDs to ensure a full profile of behavior is carefully documented and reported to all members of the treatment team.

  • Utilize tools included in “SBIRT in Schools Toolkit” to distribute to adolescents and young adults.

  • Initiate Referral to Treatment in allotted timeframe if screening results and Brief Intervention outcomes warrant it.

  • Work with Project Director to perform referrals and ensure linkage within appropriate timeframe.

  • Work with Project Director to ensure follow-up is conducted in regular intervals following referral.

  • Document and submit all patient data in appropriate systems and to appropriate members of the SBIRT team for reporting and evaluation use.

  • Maintain complete confidentiality of all patient information/data throughout 5-year program.

  • Utilize EBP techniques with full fidelity to models.

  • Collaborate with all members of SBIRT multi-disciplinary team as well as other providers to ensure a full continuum of care for all SAFER

  • Attend all SBIRT meetings.

  • Participate in all repeat trainings necessary through course of program.

What makes you a great candidate?

We can’t wait to learn more about you! Here are a few specifics of what you’ll need for the job:

  • CASAC-T (Credentialed Alcohol and Substance Abuse Counselor) Certification or higher plus a Bachelor’s Degree.

  • OR completed Master’s degree in a program that will grant licensure in the state of New York.

Location

This position will be located at 699 Hertel Ave, Buffalo NY.

Hours

This is a full-time position from Monday - Friday.

Compensation

Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $23.43 - $26.44 . Starting pay may be impacted based on experience and education. Compensation adjustments are evaluated annually.

*Please note: This is a grant-funded position.

Physical demands associated with this position include:

  • Sitting for extended periods, alternating between standing and sitting

  • Mobility required with repetitive wrist, hand, and finger movements

  • Handling light duties, occasionally lifting objects up to 20 pounds
