The Community Health Worker uses their knowledge acquired through lived experience to support improved health outcomes for individuals living with HIV across the HIV treatment cascade and care continuum. The Community Health Worker collaborates with the Retention and Adherence Program (RAP) team and members of a patient’s care team to provide strategic individual and group interventions to facilitate antiretroviral (ART) adherence and minimize barriers to reengagement and retention in care. The Community Health Worker reports directly to the HIV Treatment/Prevention Program Manager who shall determine the specific responsibilities of the position. These responsibilities can be revised, as necessary, with proper notification and training provided where appropriate.

This work is conducted in support of the New York State Department of Health Ending the Epidemic (EtE) and the Federal Health Resource & Services Administration’s Ending the HIV Epidemic in the U.S. (EHE) initiatives.

Essentials Functions:

  • Utilizes evidence-based interventions that promote adherence to ART as illustrated by the International Association of Physicians in AIDS Care (IAPAC) and the CDC’s “Partnership for Health Medication Adherence” intervention.

  • Participates in agency multi-disciplinary case conferencing on a regular basis and as needed with the Chief Medical Officer, other medical providers, and the Retention Specialists.

  • Provides linkage and referrals to appropriate wrap-around services and programs available throughout Evergreen Health and the community to address any identified barriers.

  • Monitors patient lost to care lists regularly to reengage and patients in medical care. They will assist with identifying patient barriers to retention in care and correlate services meaningfully.

  • Provides a welcoming and customer service-oriented experience for patients that is rooted in harm reduction and is trauma-informed.

  • Reviews and understands the appropriate grant work-plan from which program funding is obtained including all deliverables, required data/statistical requirements and follows all defined standards and/or guidelines as defined by the New York State Department of Health.

  • Provides all services according to the established AIDS Institute’s Retention and Adherence Services Program Guidelines including program orientation, appointment reminders, home visits, peer adherence support, etc. Expanded outreach will be completed working with the Retention Specialists.

  • Works directly with the HIV/AIDS treatment team to establish peer-based relationships with enrolled patients, serving as a liaison between the patient and medical/supportive service team. This is accomplished through phone calls, visits, encouragement of treatment, and sharing their own experiences with program participants.

  • Organizes and facilitates an adherence group for people living with HIV/AIDS.

  • Maintains complete, current and accurate patient files through the electronic medical records system (Medent) complying with agency protocols and confidentiality policies. Completes documentation of all patient encounters within the Medent system ensuring all input is current and accurate.

  • Supports health literacy skills by providing patient education in accessible language, facilitating communication among the patient and care team members, and empowering patients to make informed decisions about their health and wellbeing.

  • Actively strives to improve patient service delivery through participation in QI/QA activities working closely with quality and data team to reduce errors in reporting and to ensure data integrity.

  • Maintains a positive and respectful attitude providing professional customer service to all patients, and Association staff, always recognizing that patient needs are the Association’s top priority.

  • Attends initial orientation and training, required annual trainings, and other trainings to strengthen skills and improve program outcomes.

  • Adheres to policies regarding working hours, break periods and proper use of the Association’s ADP payroll and benefits system.

  • Maintains adherence to Association’s Corporate Compliance policies; promptly reports any violations to their supervisor, the Corporate Compliance Officer, or anonymously to the Corporate Compliance helpline at: (716) 541-0663.

  • Other duties as requested.

  • Qualified Candidate will have a High school diploma or experience in a clinical setting providing direct patient services surrounding linkage, retention and HIV viral load suppression.
