About Position:The primary responsibility of the CNA-Home Care associate is to coordinate scheduling of approved home care hours for participants and to coordinate purchase and distribution of home incontinence supplies for enrolled participants. Primary duties within these areas include tracking approved home care hours within the designated home care database for billing purposes, preparing the home care service plan for RN approval, serving as a liaison to home care agency, and ordering and overseeing distribution of home care incontinence supplies.

  • Required Travel:Yes, This positions requires frequent travel to patients homes.

Patient Population:Adult Geriatric

Duties/ Responsibilities -

  • Coordinate approved home care hours within contracted vendor network, track approved and staffed hours via home care spreadsheet for billing purposes, prepare home care service plan for RN approval and fax to contracted agency accordingly.
  • Assist in answering the phone, taking messages and/or referring to appropriate staff members. Maintain a friendly, customer service orientation at all times.
  • Order incontinence supplies and oversee distribution of supplies for all enrolled participants.
  • Serves as primary portal for all communication needed for home care service coordination.
  • Provides back up CNA support as needed in the Day Center in addressing patient care needs.
  • Demonstrates concern for the rights, privacy and confidentiality of patients and others.
  • Understands the urgency of customer needs and responds quickly.

Qualifications -

  • Ability to coordinate, plan and implement assigned projects.
  • Requires data entry/reporting ptitude.
  • Must be able to work under pressure and multi-task.
  • Exceptional customer service skills.
  • Good interpersonal, oral and written communication skills.
  • Knowledge and skill in word-processing software.
  • Must maintain confidential information and must be able to give attention to details, work independently using good judgment.
  • Problem solving skills.

Education:: HS Diploma or Equivalent; graduation from an Associate Program preferred. One year of experience with the frail or elderly preferred. 1 year experience in a customer-service/administrative assisting position in an office setting, preferably in a social service/medical facility.

Required:

  • Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment.
  • Requires frequent sitting for long periods, operation of standard office machines and computer.
  • May require lifting of up to 25 pounds. Requires eye-hand coordination and manual dexterity.
  • Requires the use of office equipment, such as computer terminals, telephones and copiers.
  • Requires normal vision.
  • Work is performed in an office environment.
  • Contact with staff and external clients/vendors.

Preferred:SPANISH SPEAKING

Current/valid License:Valid NC driver's license.

Pay Range -$17.30- $23.26
