Fairview Health Services Admissions Coordinator in Burnsville, Minnesota
We are seeking an Admissions Coordinator for our Skilled Nursing Facility -Ebenezer Ridges Care Center in Burnsville, MN! This administrative position would be responsible for developing relationships with referral sources and working with internal teams to promote and maintain census of the transitional and long term care units. You would assist the Social Services Department in meeting the needs of the senior residents and families of the care center.
This position includes;
40 hours in 2 weeks, part-time schedule
Day shift
Every other weekend

Ability to process and complete admissions
Provides pre-admission counseling, information and tours
Reviews EPIC for proper documentation and orders
Facilitates PAS Screening and Level II as needed
Ability to promote professional working relationships within Ebenezer/Fairview and with outside referral sources
Ability to assit residents with various tasks
Ability to assist Soical Service team with various tasks
Ability to participate in Vulnerable Adult inverstigations and ability to complete necessary documentation and reports
Required:
- One to three years of experience
Preferred:
Bachelor's Degree: In Medical or Human Service Field
Three to five years of experience
Two years' experience in LTC
Computer Skills
Experience in Medical or Human Service Field
