General Statement of Duties

The Ryan White Medical Case Management & Early Intervention Services Health Educator provides case management services to People Living with HIV (PLWH) along with prevention services for persons at highest risk for contracting HIV. We are looking for someone to promote individual and population health by developing and implementing health promotion and education initiatives and health behavior-change programs for the diverse populations in Butte­ Silver Bow. We are focused on strengthening community-based partnerships involving multiple public and private organizations and agencies to address the identified community health priorities.

This is position is grant funded and contingent on continued funding

Duties/Responsibilities

Essential Duties and Responsibilities

•Evaluate for eligibility of the Ryan White services & AIDS Drug Assistance Program (ADAP) and assists with application process.

•Provide initial assessment of service needs and develop a comprehensive, individualized care plan.

•Medical Case Management time must be clearly recorded in client records and monthly invoice and submitted in a timely manner.

•Timely and coordinated access to medically appropriate levels of health and support services and continuity of care.

•Continuous client monitoring to assess the efficacy of the care plan.

•Re-evaluation of the care plan at least every 6 months with adaptations as necessary.

•Ongoing assessment of the client's and other key family members' needs and personal support systems.

•Treatment adherence counseling to ensure readiness for and adherence to complex HIV treatments.

•Client-specific advocacy and/or review of utilization of services.

•In addition to providing the medically oriented activities listed, Medical Case Management will also provide benefits counseling by assisting eligible clients in obtaining access to other public and private programs (e.g., Medicaid, Medicare Part D, State Pharmacy Assistance Programs, Pharmaceutical Manufacturer's Patient Assistance Programs, other state or local health care and supportive services, and insurance plans through the health insurance Marketplaces/Exchanges).

•Provide COVID-19 and other vaccination counseling and referral based on current CDC guidelines. Counseling and referral can include answering any vaccine related questions, counseling client and/or family members on recommended vaccinations, and referring for vaccination based on CDC guidelines, state recommendations and local processes.

•Performs linkage to care activities, conducts social service referrals and follow-up.

•Teaches health literacy and navigation of the healthcare system including referral for insurance or Medicaid enrollment.

•Utilizes and maintains online resources to advertise available HIV Prevention services, Ryan White Program services & ADAP services.

•Promote and assist with access to bio-medical interventions of pre-exposure prophylaxis (PrEP) and post-exposure prophylaxis (PEP) and provide prophylactic medication adherence support.

•Performs targeted condom distribution & education throughout the community.

•Encourages community mobilization to create enabling environments that support HIV prevention by actively involving community members in efforts to raise awareness, motivate individuals to work to end the stigma related to HIV, promote health equity and encourage risk-reduction among family, friends and neighbors.

•Support the implementation of comprehensive syringe exchange services and risk reduction counseling; including rapid antibody testing, overdose prevention education and Naloxone distribution.

•Provide support and assistance for entering substance abuse treatment and/or medication assisted treatment programs.

•Collaborates with other organizations, coalitions or events relating to HIV prevention, early intervention and treatment; and other evidence-based prevention programs.

•Conduct, at minimum of 5-7 outreach activities per month, related to prevention services.

•Assist with activities related to the Mobile Medical Clinic.

•Collaborates directly with the Title X-funded Butte Family Planning Clinic for STD/HIV testing, treatment and partner services.

•Uses forms, templates and guidelines provided by the Montana Department of Public Health and Human Services.

•Follows current evidence-based guidelines issued by CDC, DPHHS, and Butte-Silver Bow County.

•Other duties as required.

Due to the need for all Health Department personnel to respond to public health emergencies, the employee must be assessed for his/her ability to meet the physical demands of performing the following activities:

1.Must be able to be fitted and wear NIOSH 95 mask.

2.Engages in the following movements: Climbing, balancing, stooping, kneeling, crouching, reaching, standing, walking, lifting, grasping, feeling and writing and repetitive motions.

3.Exerts up to 30 pounds of force occasionally, and/or a negligible amount of force constantly to move objects.

4.Hearing ability sufficient enough to communicate with others effectively in person and over the phone.

5.Visual ability must be sufficient enough to read documents, computer screens and drive a vehicle.

Working Conditions While Performing Essential Functions

1.General office setting in the Health Department facility, as well as various community locations and nontraditional settings.

2.Work performed in the community is sometimes subject to conditions that range from inclement weather to dangerous conditions such as snow/ice, cold, heat, noise, wetness/humidity, vibration, sudden temperature changes, and poor illumination at the job site or due to travel on the job. Travel to and from field locations may subject worker to increased risk of driving hazards.

Community locations may subject worker to communicable diseases, insects and other disease vectors, toxins, hazardous materials, chemicals and animals.

In all settings, employees in this classification may occasionally need to relate to members of the public who exhibit challenging, atypical or hostile behaviors and/or communication.

Equipment Used To Perform Essential Functions

Computer, fax, copier, personal or county vehicle, and various equipment and supplies.

Acceptable Education, Experience and Training

•Bachelor's degree preferred, but not required, in public health, behavioral/social sciences, clinical sciences, or a related field; or at least 3 years' experience in related field.

•Knowledge of public health education and disease prevention.

•Knowledge of HIV and interventions for both positive and at-risk individuals.

•Ability to work independently and be self-motivated (often in the field).

•Exceptional writing and interpersonal communication skills and demonstrated cultural competence are essential.

•Detailed knowledge of all current Microsoft Office software programs as well as any other software programs necessary to perform the job.

•National Incident Management System (NIMS)/Incident Command System (ICS) 100, 200, 700 and 800 certifications within six months of employment.

•HIV Counseling, Testing, and Referral System (CTRS) certification within 30 days of employment.

Required Special Qualifications

•Valid Montana driver's license, current certificate of vehicle insurance, and access to reliable transportation to all assigned work locations.

•Must be a resident of Butte-Silver Bow during employment, according to Butte-Silver Bow Ordinance 2.08.010: Residence during employment. From and after October 1, 1979, all employees of the local government of Butte-Silver Bow shall be bona fide residents within the boundaries of Butte-Silver Bow during the period of their employment by the government of Butte-Silver Bow. (Ord. 85 § 1, 1979)
