Dignity Health Market Director Community Health Outreach in Carmichael, California

Overview

Dignity Health Mercy San Juan Medical Center is a 370-bed not-for-profit Level 2 Trauma Center located in Carmichael California serving the areas of north Sacramento County and south Placer County for more than 50 years. Responsibilities

Plans, leads, and evaluates community health strategy and programs to prevent disease and promote the conditions for health. Optimizes and oversees market-wide community benefit tracking and reporting.

  1. Ensures the execution of robust and actionable community health needs assessments, implementation strategies, community benefit reports, and evaluation of impact by the hospitals and Dignity Health overall.

  2. Helps to develop community health strategy, and identifies and implements specific evidence-informed initiatives on market-wide, regional and hospital levels, in collaboration with other hospital departments.

  3. Leads market-wide tracking and reporting of community benefit programs and expenses, by working with community health, finance, and other staff in the hospitals and market offices, and ensures compliance with state and federal mandates and Dignity Health policy.

  4. Develops and maintains relationships with health, social service, and other community agencies and organizations, as well as state and national associations, to help fulfill Dignity Health's mission of service for those who are poor and disenfranchised and to improve quality of life.

  5. Manages collaboratively and coaches others to achieve optimal performance; delegates effectively; praises/rewards contributions; defines clear roles and responsibilities; sets goals and leads initiatives; adjusts plans as necessary.

  6. Shares authority and responsibility with others to move decision making and accountability downward through the organization, enabling individuals to stretch their capabilities and accomplish the business unit’s strategic priorities.

  7. Ensures that the patient/customer & community perspective is a driving force behind our actions and business decisions; crafting and implementing service practices that meet patients'/customers' and own organization’s needs. (Focus also includes internal and external customers.)

  8. Identifies opportunities and takes action to build strategic relationships between one’s area and other areas, teams, departments, units or organizations to help achieve business goals. Resolves issues and problems, and makes a significant contribution to team efforts. Keeping the organization’s mission, vision and values at the forefront of associate decision making and action.

  9. Identifies, secures and manages resources, including grants and contracts, to initiate, sustain and grow community health programs. Oversees the market’s annual Community Health Improvement Grants program in addressing priority health needs.

  10. Participates in CommonSpirit system office meetings and working groups to represent the market, share and learn strategies and tools, and help ensure standardization, efficiency and effectiveness in community health and community benefit practices.

Qualifications

Required Education and Experience:

Minimum of five (5) years of experience in community benefit and Master's degree in health administration, public health, or related field, or a combination of education and/or additional extensive, job-related experience may be substituted in lieu of the degree.

Required Minimum Knowledge, Skills, Abilities and Training:

● Demonstrates breadth and/or depth of professional/technical skills and capabilities required for position; shares knowledge; sets or contributes to

the Company’s direction within area of expertise.

● Knowledge of the policies and techniques to manage a set of related projects within a program; ability to plan, organize, monitor, and control the set of projects, ensuring efficient utilization of technical and administrative resources, to achieve program objectives.

● Understanding of how to anticipate, recognize, and deal effectively with existing or potential conflicts at the individual, group, or situation level; ability to apply this understanding appropriately to diverse situations.

● Knowledge of effective measurement techniques and ability to measure the quality and quantity of work effort for the purpose of improvement.

● Knowledge of effective planning techniques and ability to contribute to operational (short term), tactical (1-2 years) and strategic (3-5 years) planning in support of the overall business plan.

● Knowledge of and ability to apply the methods, processes and tools to conduct health needs assessment and identify the health priorities in a certain community.

● Knowledge of federal, state and local healthcare related laws and regulations; ability to comply with these in healthcare practices and activities.

● Knowledge of philosophies, approaches, processes and practices for delivering healthcare services; ability to implement, design, assess and improve healthcare service models and ensure they align with organizational goals.

Pay Range

$51.66 - $74.91 /hour
