Leech Lake Band of Ojibwe Maternal Child Health RN in Cass Lake, Minnesota

Maternal Child Health Nurse Division: Health Reports to: Maternal Child Health Program Manager Worksite: Varies Work Hours: M-F, 8 am 4:30 pm, some evenings (rarely) Safety Sensitive: Yes License/Certification Requirements: Valid RN or PHN State Nursing Licensing/Certification Required; Minnesota State Licensing/Certification preferred; CPR Certification and maintenance. Valid MN Driver s License, current insurance or be insurable. Summary: Works with women (prenatal and post-partum) at the highest risk for having poor birth outcomes; providing home visits, acting as a referral source for clients, making physical assessments, screening for risk factors and setting goals for the client with follow-up made through the first three years of the child's life. Also assists with car seat distribution. Duties/Responsibilities: Provide high quality health care services and ensure accessibility and availability of health services. Provides services for identified prenatal population of the Leech Lake Reservation. Contact mothers at high risk for poor birth outcomes through prenatal clinics, referral sources & community services. Makes physical assessments; screening for risk factors; case management, smoking status assessment, housing assessment, domestic violence assessment, depression screening and substance abuse assessment. Develop Case management plans with client input to achieve the best pregnancy outcome. Responsible for maintaining accreditation, CEU's and renewal fees, and must submit to supervisor upon each completion. Working collaboratively with appropriate entities and other agencies to ensure proper outcomes of client and/or family (Social, behavioral, legal, and other public health entities). Required to have 50-75% of the work week meeting with clientele; willing to go into client homes. Responsible for monthly reporting to supervisor as it relates to MCH Nursing. Collaborate with WIC as needed. Provide community educations & trainings: Group Sexual Education and prevention classes. Teen pregnancy prevention activities and education. Work closely with the Advocates and other LLBO departments to reinforce positive health behaviors (good nutrition, smoking cessation, breastfeeding, exercise, diabetes control, etc.). Educate and effectively communicate with those served concerning the high risk pregnancies and appropriate preventive measures and use of the health care system. Appropriate, accurate and complete medical records entries through EHR system. Assist with car seat distribution-this includes going to car seat technician training for 4-5 days. Work closely with the MCH Program Manager and other nurses in program development. Participate in local meetings, consortiums and action teams when requested. Attend and or conduct all mandatory meetings and trainings as required. Maintain confidentiality and professional ethics at all times. Travel in and out of the state on a possibly regular basis. Understanding and sensitivity to Native American culture. Adhere to policies governing staff conduct. All other related duties as assigned by Director of Nursing.
