Hazelden Betty Ford Foundation Care Coordinator in Center City, Minnesota

Care Coordinator Job Location US-MN-Center City ID 2024-11565 Status Full Time Shift Day Schedule Mon - Fri Hours of Work 8a-4:30p Responsibilities As a Care Coordinator, you will: Assist clinical case managers by working with clients, gathering, compiling and coordinating continuing care information Provide AA/NA contacts in clients home area, set up interviews, arrange transportation and contact agencies with regard to clients continuing care plan Maintain the continuing care database, with special emphasis on continuous updates of clinical value and work Work with the admissions office and provide back up for admitting clients, when needed, as well as provide the continuing care chart checks for all patients leaving treatment Follow up phone calls within 5 days of discharge for WI client facility placements for continuing care services. Qualifications Required High School Diploma or equivalent 1 or more years in customer service; intermediate level experience with Word software, including Outlook, Word, PowerPoint; in online research Preferred Vocational or associates degree Experience with patient database Basic knowledge of chemical dependency, including 12 step programs and the MN model Experience in a behavioral care setting.
