Are you driven to make a significant impact on individuals transitioning from incarceration to community life? EAC Network, a leading not-for-profit social service agency that supports over 62,888 people through 100 programs across Long Island and NYC, is seeking a dedicated Case Manager for our Transitional Case Management Substance Use Disorder (TCM SUD) Expansion Program.
Competitive Pay:$25 per hour at 35 hours per week
Schedule: 3 days in the office and 2 days working in the Suffolk County Correctional facilities. Schedule for days in the the Suffolk County Correctional facilities consist of rotating Saturday scheduled with hours of 7:30 AM-3:30 PM and one to two 12PM-8PM shifts within the Correctional facilities per week.

As a Case Manager with EAC Network, you’ll be a pivotal force in guiding individuals with substance use disorders through their transition from correctional facilities back into Suffolk County. What You'll Be Doing:
Transform Lives:Provide comprehensive transition planning and care management services to individuals with substance use disorders being released from local correctional facilities.
Fieldwork & Flexibility:Split your time between our office in Central Islip, NY, and Suffolk County Correctional facilities, with a schedule that includes rotating Saturdays and occasional evening shifts.
Collaborate for Success:Work closely with the Jail Substance Use Unit to identify and support clients, developing integrated reentry service plans that link clients to essential post-release services.
Support & Connect:Assist clients in obtaining benefits, accessing housing, and connecting with vocational, educational, and peer support services to minimize the risk of relapse or re-incarceration.
Track & Report:Monitor client progress, maintain detailed records, and ensure timely connections to community care and Health Home Care Management.
What We’re Looking For:
Qualifications:Bachelor’s Degree in Social Work, Mental Health Counseling, Criminal Justice, or CASAC with at least two years of relevant experience.
Experience:Knowledge of substance use treatment and criminal justice systems, with proven skills in collaborating with housing, treatment, and mental health services.
Skills:Excellent communication skills, both oral and written, and proficiency in Microsoft Office and other relevant software.
Other Requirements:Valid New York State driver’s license, access to an automobile, and willingness to transport clients as needed.
