Insight Global LVN Case Manager in Chatsworth, California
Job Description
Insight Global Health is looking for an Inpatient LVN Case Manager to work for a large healthcare client of ours in Irvine, CA. Their day-to-day will consist of discharge planning, post-discharge calls, reviews, contacting doctors, authorizations for patients, and coordinating care. They will be handling a caseload of between 20-25 cases a day (running high right now). This candidate must have an excellent phone presence as they will be spending a lot of time on the phone remotely. This is a fully remote position, but candidate must be able to drive to OC facilities/OC office to pick up equipment.

Skills and Requirements
2+ years of Inpatient Case Management Experience
Active LVN license in CA
Strong experience working with Microsoft Office products (Outlook, Excel, PowerPoint, Word, etc.)
Prior EMR experience
Excellent phone presence null
