Job Description Summary

The Lead Pharmacist is a key member of the multiprofessional team to provide patient-centered pharmaceutical care to aid in the recovery and rehabilitation of patients.

Job Description

The Lead Pharmacist:

  • Evaluates and optimizes pharmacotherapeutic profiles for individual patients. Collaborates with providers, nurses, therapists, and allied health professionals for the delivery of pharmaceutical care.
  • Evaluates patient\'s current pain levels (0-10) and history of recent pain experience; provides appropriate pain education and intervention in consultation with physician and pain specialists as needed; provides ongoing assessment of pain and implementation of tools for management.
  • Reviews prescription orders for correct dosing to avoid drug interactions or allergies and provides information to providers, nurses, therapists, and allied health professionals.
  • Coordinates prescriptions leaving the pharmacy for accuracy in filling/refilling medication orders, cassettes or prescriptions.
  • Assists the Director of Pharmacy in planning, organizing, implementing, directing and controlling pharmacy activities assuring conformance to standard practice of care. Monitors activities of pharmacy technicians including medication order filling, stock supplies, sterile products, prepackaging of medication, and filling prescriptions.
  • Maintains records of controlled substances used, including the monitoring, dispensing and storage in the pharmacy and nursing units.
  • Assists the Director of Pharmacy in maintaining purchasing and inventory control, as necessary.
  • Promote and facilitate adherence to formulary medications and protocols Serve as a preceptor to pharmacy students Act as a liaison between pharmacy management, operations, and clinical services as assigned by the Associate Director of Pharmacy.
  • Assists with the management of clinical pharmacy programs and services, including the development of staff and the delivery of pharmaceutical care.
  • Provides drug information services to other health care professionals including anticoagulation and pharmacokinetic monitoring of target drugs.
  • Performs all other duties that may be assigned in the best interest of the Shirley Ryan AbilityLab.

Reporting Relationships

Reports directly to the Associate Director, Pharmacy

Knowledge, Skills & Abilities Required

  • PharmD from an accredited school of pharmacy.
  • Current licensure as a Pharmacis in the State of Illinois.
  • Board Certification in applicable specialty required. 3+ years of experience as a Pharmacist.
  • Analytical skills necessary to determine drug appropriateness and evaluate drug interactions.
  • Completion of a PGY1 Pharmacy residency strongly preferred Prior hospital pharmacy experience strongly preferred.
  • Computer skills required in word processing, spreadsheets and databases.

Working Conditions

Normal office environment with little or no exposure to dust or extreme temperature.
