Shift: Shift 1

Work Schedule: 8 Hr (8:00:00 AM - 5:00:00 PM)

Summary:

The Patient Care Navigator is responsible for assisting nurses and social workers to provide outreach efforts to a group of patients from a patient list. The Patient Navigator will share knowledge and education to empower patients and improve their ability to navigate services and systems. This role will utilize the established model of care management. The individual who holds this position exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures, including complying with all Rush University Medical Center Customer Service Standards.

Required Job Qualifications:

• High School diploma or GED and 1 year in experience Electronic Medical Record

• Knowledge of ambulatory setting

• Basic knowledge of Microsoft Office

• Written, verbal and exceptional interpersonal communication skills

• Ability to multi-task

• Ability to function independently, to organize, and be self-motivated

Preferred Job Qualifications:

• Associates degree (health related area of focus such as social work, psychology or medical terminology)

• Bilingual

Required Job Qualifications:

·Previous experience in an ambulatory setting.

·Strong written and verbal communication skills.

·Exceptional interpersonal skills.

·Strong ability to multi-task in a large centralized scheduling center.

·One year experience of EMR.

·Bilingual highly desirable Proficiency with basic Microsoft Office programs.

  • Ability to function independently, to organize, and be self-motivated.

Preferred Job Qualifications:

·Associates or Bachelor’s degree.

Responsibilities:

• Daily monitoring of a list of inpatient discharges, ER discharges, and referrals.

• Provide outreach for all patients due for follow up and new patient appointments according to policy and procedures.

• Prepare and deliver routine correspondence/medical records to referring agencies and primary care

providers in accordance with policy.

• Document all correspondence in electronic medical records as necessary.

• Liaison to multiple departments, agencies and provider offices.

• Participate in meetings as assigned.

• Conduct outreach and complete assessments as necessary to identify individuals at risk for adverse health

outcomes including timely Health Risk Assessments

• Provide assistance with insurance as needed, including helping patient connect with resources for

enrollment, conducting verification of insurance plans, assisting patients with redetermination, and

others as needed.

• Coordinate transportation for all patients as needed

• Other duties as assigned

4.Monitor compliance with scheduled appointments for procedures, tests and consults.

5.Document all correspondence in electronic medical records as necessary.

6.Work to improve identified outcomes (i.e. high utilizer patients) by using quality measures.

7.Work with care team to identify extrinsic barriers to adherence (i.e. transportation, financial concerns or pharmacy) and work with patient and team to alleviate barriers.

8.Liaison to multiple departments, agencies and provider offices.

9.Participate in meetings as assigned.
