CALUMET COUNTY Human Services Professional in CHILTON, Wisconsin

JOB REQUIREMENTS: Essential Duties and Responsibilities include the following. Other duties may be assigned. Responsible for investigating reports of abuse, neglect, self-neglect, and or financial exploitation of varying types and severity while providing person-centered interventions as needed to protect the adult at risk or elder adult at risk. Advocates for the rights of adults at risk and the elder adults at risk who are residing in their own homes or community based residential facilities. Coordinates services and/or assistance for adults at risk or elder adults at risk experiencing a crisis. Represents Calumet County Department of Health and Human Services in formal court processes including filing guardianship petitions, protective placement petitions, WATTs reviews, comprehensive evaluations, and any necessary court testimony. Completes comprehensive evaluations statutorily required for purposes of protective placements/protective services. Ensures individuals are protectively placed appropriately and conducts annual reviews on court ordered placements and services in a timely manner; constantly assessing the least restrictive/most integrated setting for individuals. Responsible for state reporting requirements, professional and other supporting documentation, to ensure timely and accurate service provision. Communicates effectively and professionally with internal and external partners to ensure individuals receive needed services and supports. Ensures successful collaboration amongst key community partners with the coordination of investigations and, if applicable, an Interdisciplinary Team (I-Team). Respects the sensitivity of personal information by maintaining agency confidentiality at all times. Provides counseling, case management and coordinative services to assist clients in gaining and retaining access to services necessary to maximize their level of functioning and assure for their safety. Active participant in team collaboration meetings and professional trainings as directed. Works closely with immediate family members of the client to assist them in understanding, accepting and implementing the case plan for the client. Informs and provides assistance as needed to potential guardians to assure the mandatory training requirements for guardians is completed and filed with the court. Completes preliminary authorizations for purchased services provided on behalf of assigned cases. Refers or coordinates client case plan with other agencies providing services to meet client needs. Provides community education/consultation/intervention regarding Department services. Plays a primary role in educating the greater community and other professionals on the role of Adult Protective Services, signs of abuse, neglect, self-neglect, and financial exploitation and to ensure individuals are aware of services and supports available. Maintains case records containing pertinent, accurate and current information; prepares correspondence, reports and other records as required. Shall be assigned to perform crisis on-call duty. It is unlikely an employee will perform all the duties listed on a regular basis, nor is the list exhaustive in the sense it covers all the duties an employee may be required to perform. Supervisory Responsibilities Provides direction to professional and para-professional staff. ***** OTHER EXPERIENCE AND QUALIFICATIONS: Ability to exercise judgment and discretion in the application and interpretation of departmental policies and regulations. Knowledge of human growth and behavior; specific range of treatment modalities peculiar to the professional specialty area of the employee; social work casework principles and methods; and community based human services delivery system and resources available in the community. Ability to satisfactorily perform the following functions or demonstrate the f llowing skills including: make independent decisions; establish and maintain effective working relationships with clients, co-workers, other agencies, and the public; and provide complex services in accordance with principles common to the specialty of the employee. Ability to apply laws of confidentiality appropriately.
