GCBHS Outreach Coordinator (MH Case Worker) in Cincinnati, Ohio

Strives to develop and maintain a therapeutic relationship with the client. Makes multiple contacts as needed and provides basic needs items to engage individuals who are homeless and mentally ill. Initiates, coordinates, and monitors evaluations necessary to identify each client's needs in areas to include living arrangements, financial issues, benefit status, legal status, physical/psychiatric health, activities of daily living, vocational/educational, social supports, leisure and recreation. Serves as a liaison with community and other GCB service staff. Identifies and connects individuals served to team's mental health services aimed at addressing mental health issues as well as other basic needs, including medical services, benefits, and housing. Acts as client's CCC Position Responsibilities for Outreach Coordinators High School diploma and a minimum of three years' experience in Behavioral Health, Mental Health or a Human Services related field Bachelor's or master's degree in a Human Services field is preferred Knowledge of Homeless resources and the Homeless system is preferred GCBHS has been named a Top Workplace in Greater Cincinnati and Northern Kentucky since 2010!
