DESCRIPTION OF WORK As a Social Worker 1, you will provide social service casework and support planning that promotes patient recovery and transition back into the community. You will use working knowledge of social service and hospital policies and procedures, Centers for Medicare and Medicaid Services (CMS) regulations, current mental health legislation, and additional resources to assist patients with day-to-day coping skills. This role requires the ability to observe patient behavior in order to assess clinical needs and readiness for less restrictive treatment environments that will enable discharge planning. In addition, you will write treatment plans, document patient progress, maintain family support contact, and use state vehicles to transport patients.

Work Schedule and Additional Information: Full-time employment, 37.5 hours per week Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch. May change based on operational needs. QUALIFICATIONS Minimum Experience and Training Requirements: A bachelor's or master's degree in social work or social welfare. Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree.

Other Requirements: This particular position also requires you to possess a valid driver's license which is not under suspension. You must meet thePA residency requirement. Legal Requirements: A conditional offer of employment will require a medical examination and a drug screening. This position falls under the provisions of the Older Adult Protective Services Act. Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
