Job Description: A certified Home Nurse Assistant is a non-licensed member of the home health care team who assists the client with the daily task of living as outlined in a written care plan that is designed by a Registered Home Supervisor and is kept at the clients home. Certified Nurse Assistants are supervised by the RN supervisor and the Private day Manager at the agency and receive scheduling direction from the agencys staffing coordinator. Qualifications: 1. Must have a minimum of one year of personal care experience which is verifiable through references and must complete this agencys employment and certification process 2. Must be 18 years of age 3. Must be physically able to perform the duties of the position 4. Must exhibit mature, responsible behavior, and understand the need of the patients confidentiality 5. Must be able to follow directors and accurately report to the RN Supervisor any changes in the clients condition 6. Must have reliable and available transportation to and from the assignment Duties: 1. Duties of a Certified Nurse Assistant may include, but are limited to the following: recording information pertinent to the clients care 2. Reminding client to take medication at the appropriate time 3. Planning and preparing meals according to specific dietary requirements of the client, and if necessary feeding the client 4. Assisting the client with therapy exercise under the direction of the Registered Therapist 5. Assisting or performing personal care including hair care, oral hygiene, bathing, grooming, and dressing 6. Assisting with ambulation with or without medication 7. Assisting with routine bodily functions such as toileting 8. Maintaining a safe, clean and healthy environment through light housekeeping including changing bed linen, dusting, vacuuming, cleaning kitchens, bathroom, and laundry, performing other household tasks as indicated in the care plan 9. Performing companionship and stimulation for the client including reading, walking, and accompanying the client to the doctor and other appointments, grocery shopping and other errands as needed 10. Taking the clients vital signs and recording the input and output 11. Observing and reporting changes in the clients condition to the agency RN Supervisor 12. Accurately preparing daily records and submitting them to the office by date due 13. Participation in in-service education programs
