RN/ LPN CASE MANAGER
Position Summary
Provides professional nursing care visits to patients. Independently provides and documents patient visit. Assist with in-service and training as assigned.
Key Responsibilities and Duties
- Supervises and is responsible for PCA'S, HHA's, HMK's, ILA's in the field and provides appropriate documentation.
- Provides home care nursing visits as assigned and comprehensive client assessments in a timely manner.
- Participates in the quality improvement and utilization review activities of the organization along with any data correction procedures.
- Submits completed documentation of the home card visit within the established time parameters
- Attends multidisciplinary patient care conferences as requested.
- Attends staff meeting and in-services
- Instructs patients and caregivers according to the plan of care.
- Admits patients, develops and updates patient plan of care adhering to the STANDARD criteria for home care provision.
- Makes timely nursing assessments and reports significant changes in the patient's condition to the appropriate team members.
- Communicates to physician any changes in the patient's condition.
