Coordinator Lifestyle Intervention Participant - Strides Program

Location: METROHEALTH MEDICAL CENTER

Biweekly Hours: 80.00

Shift: 8am - 5pm

Summary:

Works closely with the STRIDES Team to deliver the STRIDES/DPP (Steps to Reach Individual Diet and Exercise Solutions/Diabetes Prevention Program) Program in both distance learning and in-person group formats. Engages referred potential STRIDES participants, coordinates participant outreach, screens for eligibility, describes program requirements, and collects baseline data while onboarding participants. Serves as liaison between new participants and STRIDES Coaching Team. Serves as technical expert for virtual lifestyle intervention platform and mobile app for both new and existing STRIDES participants. Provides technical support in a positive, encouraging manner to engage and retain socio-culturally diverse participants in the STRIDES/DPP program in order to maintain program’s credentialed status. Works closely with STRIDES Nurse Coaches and Coaches while serving as host for distance learning STRIDES classes, managing chat feature, identifying questions, and documenting attendance metrics. Coordinates email and/or mail delivery of weekly class log-in information, data collection surveys and curriculum. Coordinates provision of incentives for participant attainment of program milestones. Upholds the mission, vision, values, and customer service standards of The MetroHealth System.

Qualifications:

Required: Associate Degree or 3 years community outreach/relations experience in lieu of degree. Previous work experience in administrative support role. Demonstrated knowledge using Microsoft Office. Demonstrated ability to build strong relationships with individuals from different backgrounds and cultures and build community within a group. Must be comfortable working in a fast-paced environment. Ability to work collaboratively and flexibly and adapt to changes in the work environment. Ability to manage multiple and/or simultaneous responsibilities, and to prioritize one’s work schedule. Ability to maintain confidentiality for all medical, financial, and legal information. Ability to maintain positive working relationships with other employees and medical staff. Excellent verbal, written, and interpersonal communication skills, and excellent problem-solving skills. Ability to interact effectively with a wide range of cultural, ethnic, racial, and socioeconomic backgrounds.

Preferred: Bachelor’s Degree in a related field. Experience in group facilitation. Experience in group facilitation using a videoconferencing platform. Experience in a Disease Management Program. Experience as a health coach or health coaching certification. Experience with Epic record review. Experience with scheduling and registering functions of Epic.

Physical Requirements: May sit, stand, stoop, bend, and walk intermittently during the day. Ability to carry equipment to MetroHealth sites for weigh-in sessions (e.g. professional scale, stadiometer, materials, laptop). May need to sit or stand for extended periods. See in the normal visual range with or without correction. Hear in the normal audio range with or without correction. Finger dexterity to operate office equipment required. Ability to communicate in face-to-face, phone, email, and other communications. Ability to see computer monitor and departmental documents.
