Mitchell County Hospital District LVN-Mitchell County Nursing and Rehab in Colorado City, Texas

Position Summary:

Assists the professional nursing staff in providing maximum resident care, assessment, planning, implementation, and evaluation of each assigned resident. Under supervision gives direct and indirect resident care as assigned.

Qualifications:

Education: Graduate of an accredited School of Vocational Nursing.

Personal Job Related Skills: Effective oral and written communication skills.

Licensure, Registry or Certification: Licensed Vocational Nurse currently licensed by the State of Texas.

Experience:

  1. Prior Work Experience: One year of Licensed Vocational Nursing Preferred.

  2. Technical Training: Current CPR Certification required.

Physical and Mental Requirements: Duties of the position require extended periods of standing and walking. Bending, stooping, twisting, reaching, and lifting up to 50 pounds. Occasional heavy lifting when moving or supporting residents who may suddenly become weak or helpless.

Duties, Responsibilities, and Skills:

  1. Plans, assesses, directs and participates in the delivery of nursing home care.

o Assesses resident on admission and records the physical assessment and nursing needs upon admission and throughout stay.

o Ensure resident documentation is complete and up to date. Including vital signs, I/O, tasks, daily weights, FSG, and resident interventions.

o Identifies and documents resident/significant other teaching needs upon admission and facilitates and documents appropriate teaching and response to teaching.

o Promptly and accurately completes investigative reports as required.

o Communicates resident needs and status changes effectively with Medical Staff.

o Makes frequent resident rounds to assure resident safety, quality care, and satisfaction.

o Demonstrated knowledge of policies and procedures for administering, transcribing and recording medications.

o Demonstrates knowledge of commonly used meds: action, dose, and side effects.

o Observes seven rights for administering meds: right patient, right drug, right dose, right method, right time, right reason and right documentation.

o Demonstrates proper procedures in all methods of medication administration.

o Demonstrates knowledge of policies and procedures for medication administration.

o Maintains knowledge base through attendance at Hospital sponsored in-services for EMR.

o Maintains skills necessary to operate all Emergency equipment to ensure a safe environment for all residents.

o Assess and provides care for residents relative to their age specific needs.

o Maintains up-to-date documentation including vital signs and I/O.

  1. Evaluates the quality of nursing care in compliance with established standards of nursing practice.

o Accurately performs chart audits and facilitates education and quality improvement.

o Identifies problems with resident care and assists in formulating a plan of action to solve problems.

o Practices within legal boundaries of nurse practice act and according to hospital policy and procedures.

  1. Maintains the required inventory of supplies and equipment.

o Assures crash cart, glucometer, and maintenance logs are checked at the beginning of each shift.

o Determines equipment needs and assures equipment is maintained in condition required by department standards.

  1. Facilitates favorable relationships between the long-term care unit, Medical Staff, and other hospital departments and seeks input regarding nursing care.

o Facilitates problem resolution among peers as observed by Nurse Manager and peers.

o Demonstrates a constructive approach during all interactions with staff and Nurse Manager toward the organization.

o Interacts with resident family, other employees and departments in a considerate, helpful, and courteous manner as observed by Nurse Manager and peers.

o Fosters mature professional relationships with fellow employees in a courteous friendly manner as m asured by management observation and peer input.

o Maintains professional composure and confidence during stressful situations.

Other Significant Requirements:

  1. Maintain HIPAA required and all other confidentiality while working with confidential matters on a daily basis.

  2. Follow all policies and procedures set forth by Mitchell County Hospital District.

  3. Provides service direct to residents while maintaining a positive resident relations atmosphere.

  4. Other duties, as assigned by supervisor.

  5. Follows aseptic procedures and precautions when involved in direct resident contact or handling potentially infectious materials.

  6. Interacts with Medical Staff members, family members, and staff members of other department on a continuous basis.

  7. Participates in in-service programs, in safety, infection control, staff meetings, committee meetings, and assigned yearly education.

Work Schedule:

Rotating Schedule

Day Shift 7:00am to 7:00pm

Night Shift 7pm to 7:00am

30 minute lunch break
