Job Title: Home Health Physical Therapist
Job Description
The Physical Therapist will assist physicians in implementing and conducting treatment as per the treatment plan. The role involves monitoring the patient's response to treatment, modifying it as indicated per the plan of care and reporting to the physician or client care team. The therapist will also prepare and maintain individualized client care progress records with accuracy and timeliness as per policies. Furthermore, they will assist with interpreting and communicating evaluation findings to patients, their family and other staff members.
Hard Skills
Customer service in a one-on-one patient care in home health setting
Case management & documentation - Ability to manage/navigate case loads
Ability to complete required charting in an appropriate amount of time

Strong communication skills - Ability to communicate well with family members
Job Type
This is a Contract-to-Hire position with a duration of 6 Month(s).
Work Site
This is a position in Columbia, South Carolina requiring local travel to patient homes and client sites. Charting can be completed at home.
Work Environment
The work environment includes Home Health visits and Skilled Nursing Facilities. The position provides weekly scheduling with the flexibility to complete daily caseloads at the candidate's own pace.
