Advocates for Healthy Transitional Living Foster Care Case Manager in De Pere, Wisconsin

The Foster Care Case Manager o versee the clinical and case management services provided to foster children in their foster home, school, and community.

Requirements for Applicant:

  • Bachelor’s Degree in Social Work or related field and 2+ years’ experience, or master’s degree in Social Work or related field

  • Licensed by Department of Safety & Professional Services in Professional Counseling (LPC), Clinical Social Worker (LCSW), Advance Practice Social Worker (APSW) – 2 years’ experience required for APSW

    Primary Responsibilities :

  • Coordinate treatment team and all assessment and case reviews for each foster child on caseload

  • Review information about each child placed with foster parent, including Information for Foster Parents Parts A & B

  • Develop, in collaboration with treatment team, a comprehensive treatment plan that promotes the health and stability of the foster youth and family

  • Provide bi-weekly psychoeducation to foster families and foster youth

  • Develop and/or identify interventions and services for foster youth and family, and communicate consistently and effectively with service providers to coordinate care

  • Facilitate the development and implementation of urgent response plans for each foster child

  • Participate in supervision, team meetings and group programming

  • Ensure each child’s file strictly adheres to WI State Statutes

  • Assist in the training of foster parents, schools, and communities on the needs of children in treatment foster care

  • Perform necessary court responsibilities, as appropriate

  • Maintain appropriate and timely case records for each foster child in Kaleidacare

  • Transportation of clients as required

    Collaborative Work

  • Select appropriate foster homes for each child placed in the program (Logistics Manager)

  • Coordinate quality, evidence-based forms of service (Program Manager/Clinical Team)

  • Provide diversion/crisis stabilization (Program Manager/Clinical Team/Service Specialist)

  • Ensure clients are meeting individual goals set forth by treatment plans (Program Manager/Clinical Team/Service Specialist)

  • Communicate and collaborate with school districts to promote foster youth’s success in school when appropriate (Program Manager/Service Specialist)

  • Development and Maintenance of relationships with counties and other service partners (Program Manager/Logistics Manager)

  • Participate in intake process (Logistics Manager/Service Specialist)

  • Assign appropriate duties to Service Specialist, as needed (extra contacts, participation in Independent Living activities, etc.) (Program Manager/Service Specialist)

  • Ability to respond to customers in a timely and effective manner (staff, clients, providers, counties, etc.)

  • Hold a valid Driver’s license and ability to travel to meet customer needs when necessary

  • Appropriate knowledge and application of DHS 48, 54 and 56

  • Appropriate knowledge and application of DSM V

  • Appropriate knowledge and application of mental health disorders, substance abuse, social services systems, personal safety, HIPAA, patient rights, boundaries, and conflict resolution

  • Advanced knowledge and experience in communication, safety, and conflict resolution

  • Advanced knowledge and experience in behavioral and mental health needs of at-hope children and adolescents, Human Services systems, and Trauma Informed Care

  • Firm understanding of treatment plans and treatment issues

    Experiences and Qualifications

  • Experience with At-Hope population (preferred but not required)

  • Bachelor’s Degree in Social Work or related field and 2+ years’ experience, or master’s degree in Social Work or related field

  • Pass all background checks

  • Proof of Automobile insurance and valid driver’s license
