City Of Delta Case Manager in Delta, Colorado

POSITION: Case Manager

Under the general direction of the Crisis Prevention Unit Manager, provides case management services to support the implementation of the City's strategy to reduce homelessness, reduce noncriminal call volume, and reduce escalation of crises. Will assist the Delta Police Department (DPD), Code Enforcement, and Municipal Court by connecting residents with community resources and providing case management services.

ESSENTIAL DUTIES: *Ability to provide case management to ensure continuity of care is offered to clients and meaningful connections are made. *Ability to work directly with the Delta Police Department and Code Enforcement to meet clients at various location entry points. *Ability to work with clients at different levels of need. *Ability to engage with community partners and organizations. *When necessary, will respond on-scene with DPD to provide case management services to clients after de-escalation. *Will connect clients with community resources. *Will require work in the field. *Will assist in the coordination and facilitation of walk-in hours for case management. *Ability to respond to police radio when necessary. *Maintain detailed case notes in case management software.

REQUIREMENTS: *Must be able to pass a comprehensive background check. *Must possess at the time of appointment and maintain a valid vehicle operator's license and good driving record. *Ability to initiate and sustain relationships with staff, residents, and partners in the community. *Must be able to work autonomously as well as contribute to shared outcomes. *Excellent verbal and written communication skills, enthusiasm, creative thinking, ability to exercise independent judgment, and a commitment to excellence in customer service responsiveness. *Strong experience working with diverse populations. Understanding and knowledge of local community services and resources preferred. *Ability to work collaboratively. *Ability to understand the City of Delta Municipal Code *Schedule can include early mornings, nights, weekends, split shifts, and holidays. *Ability to adapt to situation needs.

EDUCATION and/or EXPERIENCE *Bachelor's degree in social work, psychology, counseling, or a related field. *A master's degree in social work, psychology, counseling or a related field preferred. *Licensed Social Worker preferred. A two-year degree may be substituted if the experience is equivalent. *At least five (5) years of experience in the social services field including case management experience. *A minimum of 2 years of experience in crisis interview, verbal de-escalation, and working with individuals who experience severe and persistent mental illness, and other at-risk populations. *Prior experience with Word and Excel Software packages is desirable. Ability to learn new applications.

WAGE: Range $24.03 - $28.84

SHIFT: Full time.

JOB SITE: *Requires work in the field which may include crime scenes, parks, and street outreach. *Ability to work in all weather conditions. *Ability to work safely with many distractions. *Ability to work in noisy, dirty, and wet environments. *Ability to wear PPE as required.
