Job Summary

Under minimal supervision, serves as a liaison between Adolescent Psychiatry and other Denver Health Departments to help maintain exemplary continuity of care and workflow between departments. Provides education to Adolescent Psychiatry staff as well as Denver Health physicians and other medical staff and community agencies regarding the Emergency Commitment (EC)/Involuntary Commitment (IC) laws, policies, and procedures. Establishes a learning alliance with staff to increase knowledge, skills, abilities, and attitudes. Performs culturally competent individual, family, and group therapy; care plan development; case management; and referral work within Colorado OBH Standards for clients on Emergency and/or Involuntary Commitments. Acts as a liaison between the City Attorney, the State Involuntary Commitment Coordinator(s), individuals acting as petitioner for Involuntary Commitment, and client. Coordinates physician coverage for IC clients admitted to Adolescent Psychiatry. Participates in policy planning and implementation, and creation of standard work. Essential Functions:

Provides direct service to patient populations served and their families. (5%) Responsible for bio-psycho-social evaluation/ intake, designs treatment recommendations and creates service plans, at time of admission. (5%) Ensures evaluations and service plans cover a broad range of patient functioning and serve as the clinical justification for admission into the recommended level of care. (5%) Ensures all documentation clearly addresses the patient's individual life areas, in need of clinical support, to achieve positive clinical outcomes. (5%) Facilitates both educational and therapeutic groups as well as individual sessions and fundamental case management. (5%) Works as a member of a multidisciplinary patient care team demonstrated by the capacity to ask for additional tasks, or helping team members when not busy. (5%) Coordinates medication/medicinal needs of patients with medical sources. (5%) Responds to patient and referral needs in a timely, professional and non-defensive manner. (5%) Documents legibly, while accurately reflecting the patient's clinical progression. (5%) Ensures all therapeutic interventions and patient response to interventions are documented within the prescribed guidelines. (5%) Ensures all service updates and documentation are completed per established state and agency guidelines. (5%) Coordinates patient billing and modifies treatment program in accordance with insurance coverage, plan provisions and individual ability to pay. (5%) Helps maintain a safe environment for patients by responding to staff supports. (5%) Utilizes a variety of crisis intervention techniques, such as CPI to respond to aggressive behavior. (5%) Assists with filling in-service needs as requested. (5%) Makes attempts to re-engage any client who seeks a premature discharge from the program. (5%) Provides administrative support and assists in developing, implementing and interpreting policies, procedures, and programs. (5%) Routinely communicates with the Addictions Supervisor regarding staff, services, activities and problems needing departmental attention. (5%) Delegates authority and responsibility, as appropriate. (5%) Provides Adolescent Psychiatry staff education on evidence-based practices in working with clients with dual diagnoses in a detox setting. (5%) Routinely meets with physicians for review of active cases, problem solving, continuity of care, ongoing communication, and consultation. (0%) Coordinates with Psychiatric Emergency Services with clients under an Emergency or Involuntary Commitment to ensure continuity of care. (0%) Identifies new and changing resources and programs in the community in order to provide services to patients, negotiates referral and application procedures for these services, and facilitates exchange of information between community agencies and staff.
