Henry Ford Health System EMT-Basic Tech - Mobile Integrated Health Department- Full Time (40 Hours) in Detroit, Michigan

Mobile Integrated Healthcare is the provision of healthcare using patient-centered, mobile resources in the out-of-hospital environment (American College of Emergency Physicians, 2020). At Henry Ford Health, Mobile Integrated Health is an integrated service designed to enhance a patient's experience by providing comprehensive, safe and efficient in-home care as an alternative to an emergency department visit or hospitalization.

The Mobile Integrated Health EMT-Basic Technician for Population Health Management is a member of the care team who is responsible for supplementing and supporting the care provided by the Mobile Integrated Health Medic and/or physician; by performing follow-up assessments, and diagnostic testing under the direction of a physician and/or MIH Medic in a patient's home or current residence.

    PRINCIPLE DUTIES AND RESPONSIBILITIES:

  • Conducts an evaluation of the patient’s medical and social history to prepare for follow-up care and/or testing plan.

  • Conducts an in-home assessment of the patient which includes but is not limited to environmental safety and an evaluation of social support systems.

  • Conducts an evaluation of the patient’s medical and social history to support care recommendations that optimize care transitions to the physician and other members of the care team.

  • Provides diagnostic testing in the home under the direction of a physician and/or an MIH Medic. Interventions and diagnostic tests include but are not limited to blood draws, specimen collections and ECGs.

  • Conducts testing and examinations with medical diagnostic tools such as a virtually enabled diagnostic tool which includes an otoscope, stethoscope, tongue blade and other useful attachments.

  • Promotes healthy behaviors through education and counseling.

  • Promotes cross functional communication with multiple care teams such as other MIH Service members, ambulatory clinic, case management, home health care agency, skilled nursing facility, community organization and health plan personnel by regularly communicating with care teams.

  • Engages patients through advocacy, ongoing communication, health education, identification of service resources and service facilitation.

  • Participates in process and quality improvement activities designed to improve workflow and patient outcomes. May include data collection, chart review, and participation in case studies.

  • Performs other duties as assigned.

    EDUCATION/EXPERIENCE REQUIRED:

  • Minimum Three (3) years of clinical experience.

  • Community Health Worker experience/certification desirable.

  • Certification in phlebotomy desirable.

  • Experience in a medical office or hospital setting desirable.

  • Knowledge of anatomy and pathophysiology as well as chronic conditions and implications in high-risk populations.

  • Extensive understanding of community resources and the needs of diverse populations.

  • Excellent customer service and interpersonal skills.

  • Strong problem solving, analytical and decision-making skills.

  • Strong organizational, planning and implementation skills. Able to balance multiple tasks simultaneously.

  • Excellent written and verbal communication and documentation skills.

  • Adaptable to a changing work environment.

    CERTIFICATIONS/LICENSURES REQUIRED:

  • Licensure as an Emergency Medical Technician – Basic or Emergency MedicalTechnician - Intermediate, with a valid State of Michigan license.

  • American Heart Association - Basic Life Support (BLS) certification.

  • Valid State of Michigan Driver’s License.

    PHYSICAL DEMANDS/WORKING CONDITIONS:

  • Works in a patient care environment with exposure to communicable diseases.

  • Manual dexterity is required to complete the performance of duties.

  • Walk, stand, kneel, stoop, and may require some heavy lifting of patients and/or equipment. With an estimated

  • push and pull 35- 50 lbs. on a wheeled cart approximately 2-3 times in a shift.

  • Carry medical equipment into homes with stairs with an approximate weight of 25lbs.

  • Paramedics / EMT-Bs who work in home care environments will be required to drive on surface streets/highways • and be subject to variable weather conditions.
