Summary: UNC Hospitals (UNCH) Department of Pharmacy has an opening for a motivated pharmacist interested in joining our clinical oncology clinical pharmacy team in the Chapel Hill, NC region. The focus of this position will be on providing comprehensive clinical pharmacist care services, disease management and specialty pharmacy support to adult patients seen in the outpatient hematology and medical oncology clinics at UNC Hospitals. This individual will ensure continuity of care as the patient transitions between acute care and ambulatory care settings, while continuing to pursue best practices in this area. Additionally, the individual in the position will develop preceptorship and research skills through experiential instruction to Doctor of Pharmacy Students and UNC Medical Center pharmacy residents.
This individual will directly report to the Clinical Manager of the Hematology/Oncology Service Line. Additionally, this individual will work closely with the Hematology/Oncology Clinical and Central Specialists as well as the Specialty Pharmacists within the UNC Carolina Care Pharmacy Care Network.
Monday - Friday daytime hours, 8:00 am - 4:30 pm. Variable schedule meaning the position can cover medical oncology and hematology clinics, and the pharmacist would support the different clinics depending on needs.
ESSENTIAL DUTIES AND RESPONSIBILITIES
This individual will provide comprehensive pharmacists care services, and optimizing medication use for patients seen in the outpatient hematology and medical oncology clinics through innovative clinical services, ensuring continuity of care by bridging acute and ambulatory pharmacy services, and leading quality improvement initiatives. This is a Monday to Friday day shift position with flexibility for some remote scheduling.
Extensive clinical skills and pharmacotherapy knowledge base in Hematology/Oncology. Equivalent experience is defined as clinical experience in hematology oncology clinical pharmacy practice
Strong oral and written communication, organizational, and prioritization skills
Ability to work in team environment
Ability to generate meaningful contributions related to hematology/oncology and advancement of pharmacy practice.
Ability to design and implement projects and collect and interpret data accurately
Clinical responsibilities:
Manage an outpatient hematology/oncology clinic
Collaborate with physicians and advanced practice providers to provide comprehensive patient care
Medication Therapy Management
Educate patients and their caregivers about their chemotherapy plan
Manage chemotherapy toxicities
Provide comprehensive medication management for patients receiving oral chemotherapy agents
Coordinate medication related aspects of transitional care
Collaborate with the acute care pharmacists and cancer hospital infusion pharmacy team caring for the hematology/oncology patients on pharmacy related policies, procedures, and guidelines
Support medication management goals set forth by the institution or Department of Pharmacy
Serve as a liaison between the Department of Pharmacy and the Division of Hematology/Oncology to assure all activities are coordinated and consistent
Serve as a clinical liaison among the clinical oncology team, UNC Medication Assistance Program, and specialty pharmacies (including UNC Shared Services Center Pharmacy) to help address medication access barriers and ensure timely processing and delivery of specialty medications
Service Responsibilities
Serve as a standing committee member on a select group of committees within UNC Medical Center
Contribute to ongoing quality assurance initiatives for the Department of Pharmacy, the Hematology/Oncology Division, and UNCH
Educational Responsibilities
Educate students from the UNC Eshelman School of Pharmacy and PGY1 and PGY2 residents from UNC Medical Center on their experiential rotations in adult hematology/oncology rotations
Appointment at UNC Eshelman School of Pharmacy is available with commensurate experience

Education Requirements:
PharmD or BS Pharmacy Degree
Completion of a Pharmacy Practice (PGY1) Residency or equivalent experience
Completion of an Oncology (PGY2) Pharmacy Residency or equivalent experienceBoard Certification in Oncology Pharmacy
Licensure/Certification Requirements:
● Active pharmacist licensure with the NC Board of Pharmacy or obtainable before initiation of employment. Recent graduates of ACPE-accredited colleges/schools of pharmacy may fill this role while pending initial pharmacist licensure, but are subject to limitations mandated by the NC Board of Pharmacy and must obtain licensure within 6 months of orientation or sooner if required for operations.
