UNC Health Care Respiratory Therapist II - Homecare in Durham, North Carolina

Summary:

Under the supervision of UNC Homecare Specialists Department Management/Respiratory Supervisor, the Respiratory Therapist has the responsibility to collaborate with other members of the healthcare team to carry respiratory and sleep therapy patients through the continuum of care for best possible outcomes. Respiratory Therapist works in a collaborative effort with providers, case management and patients’ nursing care team as needed and is a resource to provide appropriate respiratory homecare durable medical equipment and supplies. Respiratory Therapist takes an active role in facilitating communication of patient’s needs, order requirement for DME, and provides feedback to medical team to avoid unnecessary delays in care, functional improvement, timely discharge, in attempt to avoid readmissions. Respiratory Therapist demonstrates desire to promote the company philosophy and organizational goals providing Carolina Care.

Key duties include:

Maintains an environment of safety for patients, self, and others.

Implements routine respiratory therapy procedures as prescribed by a physician/medical provider. Deliver, set up, pick up, educates patient/caregiver on the use of supplies and durable medical equipment (DME).

Performs conserving device/pulse dose evaluations per MD order with appropriate patient safety needs in mind. Documents using the appropriate standardized charting template in EMR. Communicates with provider any negative results.

Delivers and instructs patients on proper use of oximetry equipment as per MD orders. If applicable, downloads data, faxes MD report with results. Documents using standardized template in EMR.

Evaluates and reviews order for Trach/Suction equipment for accuracy. Creates delivery ticket, delivers and instructs patient/caregiver on the safe use of equipment and supplies per MD order. Documents using the standardized templates in EMR, and collaborates appropriate interdepartmentally to coordinate set up/delivery.

Evaluates and reviews PAP/NIV therapy orders for accuracy. Sets PAP/NIV therapy parameters per MD orders and educates patient/caregiver on the proper use of equipment. Assures appropriate interface and fitting to meet the need of the individual patients. Assures patient is able to apply interface and use effectively. Documents in EMR using standardized template for PAP/NIV therapy. Communicates with patients for timely follow-up utilizing To Do’s, and note manager process to schedule weekly, 30,60,90-day follow-up with patient as needed as well as monthly Astral vent. check as needed. Follows process for communication to providers notifying of set-up, and face-to-face follow-up needed, and compliance status using form letter and EPIC in-basket. Participates in departmental performance improvement plans. ' Utilize EMR (CPR+) to review and/or create delivery tickets using appropriate template, progress note template, notes manager reports, and To Do process.

Utilize UNCHCS electronic medical record CPR+ for documenting all RT patient visits and communication, utilizes EPIC electronic medical record for clinical view or progress note entry as needed.

Ensures deliveries are on time and correct. Communicates with patients timely if scheduling conflict arises. Provides timely follow up with patients as per departmental compliance guidelines. Manages territory case managing patients. Calls patients timely to schedule appointments and arrival time. Limits patient/agency complaints about deliveries from patients or caregivers.

Meets education requirements, reviews and updates competencies per regulatory requirements.

Displays a clean, neat, and professional appearance at all times. Complies with dress code 100% of the time. Maintains attendance standards by following assigned work hours, limiting absences, and providing proper absence notification. Follows organization attendance policy. Follows clock in/out procedure, use time wisely, limits overtime. Participates in on-call rotation. Responds timely to needs when on-call. Coordinates appropriately with dispatch, CSR, delivery technicians, and/or intake CSR to arrange deliveries if needed.

Documents outcomes in patient EMR. Performs routine troubleshooting of equipment to assess needs of patient. Provides education and instruction to patient/caregivers on related home care respiratory equipment being provided.

Communicates patient responsibility with DME provided, integral follow-up, communicates any home delivery needs, as well as resupply/inventory needs. Educates patient and/or caregiver on proper use, cleaning, troubleshooting of RT equipment prior to discharge or in home setting. Ensures regulatory paperwork is completed accurately, reviewed, and submitted timely. Contacts patients to maintain scheduled appointments and avoid rescheduling RT visit whenever possible. Attends department meetings and training in-services.

Other related duties as assigned.

Maintains professional behavior and communication at all times and follows organization code of conduct. Operates work vehicle in appropriate and safe manner following organizational and regulatory guidelines.

Education Requirements:

● Graduation from an accredited Respiratory Therapy program.

Licensure/Certification Requirements:

● Current certification as a Registered Respiratory Therapist (RRT) by the National Board of Respiratory Care (NBRC) required. Note: Current teammates with active Certified Respiratory Therapist (CRT) certification by the NBRC and a minimum of 15 years of experience as a CRT and are ineligible for the RRT board exam with demonstrated proficiency in all aspects of respiratory care also qualify for this role.

Dependent upon population served and regardless of if RRT or CRT, all teammates must have at least one of the following depending upon practice area: Advanced Cardiac Life Support (ACLS) – Adult Care , Neonatal Resuscitation Program (NRP) - NICU, Pediatric Advanced Life Support (PALS) - PICU/Rex, Certified Pulmonary Function Technologist (CPFT), or advanced practice certificate to administer moderate sedation in Bronchoscopy Lab. All certifications must be maintained as required by the certifying body.

Professional Experience Requirements:

● Two (2) years of experience as a Respiratory Therapist

Knowledge/Skills/and Abilities Requirements:

● Ability to read, analyze, and interpret clinical information and technical procedures. Ability to document clinical information. Ability to effectively communicate information and respond to questions from patients, physicians, family members, and other staff. Mathematical Skills: Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations. Reasoning Ability: Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical situations.
