Position Summary
The Michigan State University (MSU) College of Nursing (CON) is seeking an experienced registered nurse (RN) with an interest in chronic care management to enhance diabetic and pre-diabetic patient outcomes for diverse populations in a Nurse Practitioner-led clinic. The RN will be a part of a collaborative team of nurses providing high-quality care that supports inclusive and culturally appropriate behavior modifications and is expected to function at the top of their scope of practice. As a part of the team, the RN will use a scholarly approach, which indicates their understanding of inclusive patient care in diverse communities, making use of evidence-based practices. The RN will use continuous quality improvement processes to assure equity, quality, safety, and enhanced patient outcomes in diverse populations. The successful RN candidate must be certified as or eligible to be certified as a Diabetic Care & Education Specialist (DCES). The selected candidate will also serve as a preceptor to RN nursing students for the MSU College of Nursing.

Responsibilities include but are not limited to:
Demonstrate the knowledge and skill necessary to complete initial patient/family educational sessions, assessments, reassessments and follow-up, and suggest referrals as necessary with accuracy and skill
Recommend treatment changes to enhance diabetic patient outcomes and adherence to treatment
Educate diabetic and prediabetic patients and their family members on appropriate care, management, and treatment
Focus on collaborative work and informed patient care to ensure that patients make realistic lifestyle changes to enhance outcomes
Create and maintain inclusive and equitable partnerships and collaborate to enhance patient outcomes
Serves as a nutrition and diabetes consultant to the healthcare team
Assemble information concerning patient's clinical background and referral needs
Utilize critical thinking in determining alternative solutions and assist in solving patient problems and issues as need to be in compliance with diabetic disease
Utilize American Diabetic Association (ADA) and American Association of Diabetic Educator (AADE) approved curriculum and education materials for patient instruction
Utilize appropriate motivational interviewing techniques necessary for coaching and assisting the patient to complete self-management goal/action plans
Collaboratively develop goals, learning objectives and a plan for educational content and teaching methods
Provide education interventions that utilize interactive, skill-based training methods for group classes and individual teaching
