WORKFORCE RESOURCE INC Independent Living Career Planner in EAU CLAIRE, Wisconsin
JOB REQUIREMENTS: This is a combined position of Independent Living and Anti-Human Trafficking. This program will be working with young adults age 14-22 that have been human trafficked/aged out of foster care to aid in their goals and assist them with healthy relationship, employment, education, housing, and community resources. Staff will be responsible for working checking program eligibility, working with county and community partners, and work on long term case management. This position will do both face-to-face appointments and use remote communication technology. Staff will assist in meeting state standards, data collection and recruitment of youth to attend and lead life skills courses and support groups. Specific responsibilities include: 1. Direct services for youth ages 17.5-23 o Providing primary direct services and case management for eligible young adults between the ages of 18-23 beginning with transition planning at age 17.5 in partnership with county and/or Indian Child Welfare Workers. o Providing primary direct services working with youth who have been human trafficked until age 21. o Streamlining access to and increasing utilization of housing, employment, positive social bonds, education, youth engagement in their community, positive youth development, and healthcare services, as needed and directed by each youth. o Providing supplemental services and case management, as needed, for eligible youth, young adults whose physical, mental, and/or emotional needs warrant primary service provision by another program area (e.g. adult services). o Connecting youth pursuing postsecondary education to ETV funding, WIOA programing, FAFSA and other postsecondary resources. o Work toward co-enrollment of youth within the agency for WIOA/FSET/W-2/Support to Communities and other relevant career services o Screening youth for mental health traumas, and if they qualify for Anti-Human Trafficking (AHT) supports. o Case manage youth who have been trafficked, if qualified for AHT. o Keep confidential records and case notes in an orderly manner. o Expense out client requests to financial department in timely manner. 2. T-SBIRT screening of youth going into services at Workforce Resource: Screen all youth under 21 for Trauma to determine qualification of AHT programing Refer youth to services within the agency such as W2 and TJ etc 2. Direct services for youth ages 18-21 o Providing primary direct services and case management for eligible young adults between the ages of 18-21. o Streamlining access to and increasing utilization of housing, employment, positive social bonds, education, youth engagement in their community, positive youth development, and healthcare services, as needed and directed by each youth. o Connecting youth pursuing postsecondary education to FAFSA and other postsecondary resources. o Work toward co-enrollment of youth within the agency for career services and other necessary resources. o Keep confidential records and case notes in an orderly manner. o Expense out client requests to financial department in timely manner and deeming what reasonable and necessary for youth\'s program goals. 3. Partnership with regional network of resources for youth o Collaborate with community resources such as local service agencies, educational institutions, county and tribal welfare agencies, and local employers to address needs related to employment, education, housing, health, positive social bonds, finances and financial literacy, and other areas of self-sufficiency. o Convene the network on a regular basis to encourage collaboration. o Participate in/support the regional Youth Advisory Board and encourage youth to have meaningful input into program design. o Recruit and assist with the Multidisciplinary Team Meetings on a monthly basis. 4. Implement meetings for youth advocacy, healthy relationships, and life skills Recruit youth on case load to have monthly me tings for No More Silence to have opportunities ***** OTHER EXPERIENCE AND QUALIFICATIONS: Skills in leadership, public speaking; knowledge of community resources; knowledge of regional businesses and the economy in the geographic area assigned; effective interpersonal and counseling skills; group facilitation and organizational skills; ability to keep accurate records; ability to travel.
